摘要
目的:探讨Dickkopf-1(DKK-1)、血管内皮生长因子(VEGF)与乳腺癌临床特征和预后的关系。方法:选取2014年6月至2016年6月青岛大学附属医院收治的109例乳腺癌患者为研究对象。免疫组织化学法检测乳腺癌组织及癌旁组织中DKK-1、VEGF的表达,分析二者与乳腺癌临床特征间的关系。所有患者出院后随访60个月。Spearman法分析乳腺癌组织DKK-1、VEGF的关系。Kaplan-Meier法分析DKK-1、VEGF与乳腺癌患者预后的关系。COX回归分析影响乳腺癌患者预后的危险因素。结果:与癌旁组织比较,乳腺癌组织DKK-1、VEGF阳性表达率均显著升高(P<0.05)。乳腺癌组织中DKK-1、VEGF阳性表达与患者TNM分期、淋巴结转移有关(P<0.05)。乳腺癌组织中,DKK-1阳性表达与VEGF阳性表达呈正相关(r=0.372,P<0.05)。DKK-1、VEGF阳性表达组患者5年内生存率(51.3%、53.4%)均低于阴性表达组(81.4%、74.5%)。多因素COX分析表明,淋巴结转移、DKK-1、VEGF阳性表达是影响乳腺癌患者预后的危险因素(P<0.05)。结论:DKK-1、VEGF表达可能与乳腺癌预后有关,可能是乳腺癌潜在预后标志物和治疗靶点。
Objective:To detect the expression of Dickkopf-1(DKK-1)and vascular endothelial growth factor(VEGF)in breast cancer,and analyze the relationship between them and clinical features,prognosis of breast cancer.Methods:A total of 109 cases of breast cancer patients in our hospital from June 2014 to June 2016 were selected as the research objects.Immunohistochemistry was used to detect the expression of DKK-1 and VEGF in breast cancer tissues and adjacent tissues,and the correlation between DKK-1 and VEGF and clinical features of breast cancer patients was analyzed.All patients were followed up after discharge,and their survival status was recorded.The follow-up time was 60 months.Spearman method was used to analyze the correlation between DKK-1 and VEGF in breast cancer tissues.Kaplan-Meier method was used to analyze the relationship between DKK-1,VEGF and prognosis of breast cancer patients.COX regression analysis was used to analyze the risk factors influencing the prognosis of breast cancer patients.Results:The positive expression rates of DKK-1 and VEGF in breast cancer tissues were significantly higher than those in adjacent tissues(P<0.05).The positive expression of DKK-1 and VEGF in breast cancer was related to TNM stage and lymph node metastasis(P<0.05).There was a positive correlation between the positive expression of DKK-1 and VEGF in breast cancer(r=0.372,P<0.05).The 5-year survival rate of DKK-1 and VEGF positive expression groups(51.3%and 53.4%)was lower than that of negative expression groups(81.4%and 74.5%).Multivariate COX analysis showed that lymph node metastasis,positive expression of DKK-1 and VEGF were risk factors for prognosis of breast cancer patients(P<0.05).Conclusion:The expression of DKK-1 and VEGF in breast cancer may be related to the prognosis of patients,and they may be potential prognostic markers and therapeutic targets of breast cancer.
