摘要
目的分析偏头痛合并卵圆孔未闭患者应用经颅多普勒发泡试验(transcranial Dopplerbubble test,TCD)诊断的效果。方法纳入2019年4月—2020年7月收治的偏头痛合并卵圆孔未闭患者21例为研究对象。对这些研究对象依次实施二维超声心动图检查和TCD。以经食管超声心动图(transesophageal echocardiography,TEE)为诊断卵圆孔未闭的“金标准”。分析偏头痛合并卵圆孔未闭检查准确率、漏诊率、误诊率。结果TCD诊断偏头痛合并卵圆孔未闭的准确率高于二维超声心动图,差异有统计学意义(P<0.05)。TCD诊断偏头痛合并卵圆孔未闭的漏诊率与二维超声心动图差异无统计学意义(P>0.05)。TCD诊断偏头痛合并卵圆孔未闭的误诊率低于二维超声心动图,差异有统计学意义(P<0.05)。结论偏头痛合并卵圆孔未闭患者应用TCD诊断的临床价值更加显著,结果可靠,且TCD本身具有安全、无创等优势,临床筛查价值较高,具有临床推广价值。
Objective To analyze the effect of transcranial Dopplerbubble test(TCD)in diagnosis of migraine complicated with patent foramen ovale.Methods A total of twentyone patients with migraine combined with patent foramen ovale who were treated from April 2019 to July 2020 were included as the research objects.Two-dimensional echocardiography and TCD were sequentially performed on these subjects.Transesophageal echocardiography(TEE)was the"gold standard"for the diagnosis of patent foramen ovale.The accuracy,missed diagnosis and misdiagnosis rate of migraine combined with patent foramen ovale were analyzed.Results The accuracy of TCD in diagnosing migraine with patent foramen ovale was higher than that of two-dimensional echocardiography,and the difference was statistically significant(P<0.05).The missed diagnosis rate of TCD in the diagnosis of migraine complicated with patent foramen ovale was not significantly different from that of two-dimensional echocardiography(P>0.05).The misdiagnosis rate of TCD in the diagnosis of migraine complicated with patent foramen ovale was lower than that of two-dimensional echocardiography,and the difference was statistically significant(P<0.05).Conclusion The clinical value of TCD in the diagnosis of migraine patients with patent foramen ovale is more significant and the results are reliable.TCD itself has the advantages of safety and noninvasiveness,and has high clinical screening value and clinical promotion value.
