摘要
目的:舒肝解郁胶囊辅助治疗胃癌前病变对血清胃泌素和胃蛋白酶原及病变进展程度的影响。方法:回顾性收集2016年2月到2017年6月于我院就诊的90例胃黏膜肠上皮化生患者。根据治疗方案不同,分为对照组和观察组,每组45例,对照组采用雷贝拉唑肠溶片等治疗,观察组在对照组基础上采用舒肝解郁胶囊辅助治疗。于治疗前与治疗第1、2、3、4、5 y对比两组患者血清胃泌素17水平(Gastrin 17,G-17)、胃蛋白酶原(Pepsinogen,PG)水平、胃蛋白酶原受体(Pepsinogen receptor,PGR)水平、内镜下病变进展程度及病理组织学变化。结果:治疗前,治疗第1、2、3 y,两组同年血清G-17、PG-I、PG-II、PGR水平无显著差异(P>0.05)。与对照组比较,治疗第4、5 y,观察组G-17、PG-II明显降低,PG-I、PGR明显升高(P<0.05);观察组内镜下和病理学表现病情恶化发生进展的比例明显降低(P<0.05)。结论:舒肝解郁胶囊辅助治疗胃黏膜肠上皮化生具有延缓或抑制病情进展的作用。
Objective:To investigate the effect of Shugan Jieyu capsule on serum gastrin and pepsinogen and the progression of gastric precancerous lesions.Methods:90 patients with intestinal metaplasia of gastric mucosa treated in our hospital from Feb.2016 to Jun.2017 were collected.According to different treatment schemes,they were divided into control group and observation group,with 45 cases in each group.The control group was treated with rabeprazole enteric coated tablets,and the observation group was treated with Shugan Jieyu capsule based on the control group.The levels of serum gastrin 17(G-17),pepsinogen(PG),pepsinogen receptor(PGR),microscopic lesion progression and histopathological changes were compared between the two groups before and after treatment.Results:There was no significant difference in serum G-17 and PG levels between the two groups before treatment and after 1,2 and 3 years of treatment(P>0.05).Compared with the control group,at the 4th and 5th years of treatment,G-17 and PG-IIin the observation group were significantly reduced,PG-IIand PGR were significantly increased(P<0.05);the worsen proportion of endoscopic and pathological manifestations in the observation group were significantly decreased(P<0.05).Conclusion:Shugan Jieyu capsule can delay or inhibit the progression of gastric mucosal intestinal metaplasia.
