^(99)Tc^(m)-3PRGD_(2)单光子发射计算机断层显像在大鼠乳腺癌骨移植瘤放疗疗效评价中的价值观察

Value of^(99)Tc^(m)-3PRGD_(2)single photon emission computed tomography in evaluation of radiotherapy efficacy in rats with breast cancer bone grafts

摘要 目的观察^(99)Tc^(m)-3PRGD_(2)单光子发射计算机断层(SPECT)显像在大鼠乳腺癌骨移植瘤放疗疗效评价中的价值。方法50只Wistar大鼠通过骨髓腔内注入含有大鼠乳腺癌细胞的腹水混悬液建立胫骨移植瘤模型,最终成瘤30只。将30只乳腺癌骨移植瘤大鼠按随机数字表法分为对照组、低剂量放疗组、高剂量放疗组各10只,分别于骨移植瘤部位行0 Gy、5 Gy、25 Gy的单次大剂量直线加速器放疗,各组分别于放疗前及放疗结束后第10天尾静脉注射^(99)Tc^(m)-3PRGD_(2),1 h后行SPECT显像并计算骨移植瘤部位放射性计数与对侧相应部位同等大小区域的放射性计数比值(T/NT)。显像结束后处死大鼠,采用HE染色观察大鼠胫骨组织病理改变,采用免疫组织化学法观察骨移植瘤组织整合素αⅤβ_(3)受体表达情况。采用Spearman相关性分析T/NT与整合素αⅤβ_(3)受体阳性细胞表达的关系。结果各组放疗前行^(99)Tc^(m)-3PRGD_(2)SPECT显像,骨移植瘤部位均可见显像剂浓聚影,浓聚程度无明显差异;各组放疗前T/NT比较无统计学差异。各组放疗后行^(99)Tc^(m)-3PRGD_(2)SPECT显像,显像剂浓聚程度对照组>低剂量放疗组>高剂量放疗组;各组放疗后T/NT对照组>低剂量放疗组>高剂量放疗组(P均<0.05)。HE染色可见各组胫骨组织均有骨结构破坏,对照组内肿瘤细胞密度高于高、低剂量放疗组,高、低剂量放疗组内有大量肿瘤细胞坏死,其中以高剂量放疗组肿瘤细胞坏死最多。对照组、低剂量放疗组、高剂量放疗组骨移植瘤组织整合素αⅤβ_(3)受体阳性率对照组>低剂量放疗组>高剂量放疗组(P均<0.05)。Spearman相关性分析显示,T/NT与αⅤβ_(3)受体阳性细胞表达呈正相关(r=0.847,P<0.01)。结论^(99)Tc^(m)-3PRGD_(2)SPECT显像指标变化与骨移植瘤病理改变一致,用于大鼠乳腺癌骨移植瘤放疗疗效评价的效果良好。 Objective To observe the value of^(99)Tc^(m)-3PRGD_(2)single photon emission computed tomography(SPECT)imaging in evaluating the efficacy of radiotherapy in rats with breast cancer bone xenografts.Methods We in-jected ascites suspension containing rat breast cancer cells into the bone marrow cavities of 50 Wistar rats to establish the tibial graft tumor models,and finally 30 Wistar rats were successful.Thirty breast cancer rats with bone graft tumor were randomly divided into the control group,low-dose radiotherapy group,and high-dose radiotherapy group,with 10 rats in each group.The rats were treated with linear accelerator radiotherapy of 0 Gy,5 Gy and 25 Gy at the site of bone graft tu-mor for 1 day,respectively.In each group,^(99)Tc^(m)-3PRGD_(2)was injected into the tail vein before radiotherapy and on the 10th day after radiotherapy,respectively.PECT imaging was performed 1 h after injection,and the ratio of the radiation count of the bone graft tumor site to the same size area of the corresponding contralateral site(T/NT)was calculated.After imaging,the rats were sacrificed and HE staining was used to observe the pathological changes of rat tibia,and immunohis-tochemistry was used to observe the expression of integrinαⅤβ_(3)receptors in bone transplanted tumors.Spearman correla-tion was used to analyze the relationship between T/NT and the expression of integrinαⅤβ_(3)receptor-positive cells.Re⁃sults Before radiotherapy in each group,^(99)Tc^(m)-3PRGD_(2)SPECT imaging was performed,and the imaging agent concentra-tion was seen in the bone graft tumor.The degree of concentration had no significant difference and there was no statistical difference in T/NT before radiotherapy in each group.After radiotherapy,^(99)Tc^(m)-3PRGD_(2)SPECT imaging was performed in each group.The concentration of imaging agent in the control group was higher than that in the low-dose radiotherapy group,and that in the low-dose radiotherapy group was higher than that in the high-dose radiotherapy group.The order of the T/
