摘要
目的探讨诊断性刮宫在围绝经期及绝经后阴道出血诊断中的应用价值。方法选择本院收治的100例阴道出血患者为研究对象,按照患者的实际情况将其分为两组,围绝经期组50例,绝经后组50例,所有患者均需要接受诊断性刮宫,分析检查结果。结果围绝经期组患者检出炎症10例(20%)、非典型性增生18例(36%)、功能性子宫出血16例(32%)、复杂性增生19例(38%),绝经后组患者检出炎症12例(24%)、非典型性增生19例(38%)、功能性子宫出血14例(28%)、复杂性增生20例(40%),两组的各病因检出率比较,差异无统计学意义(P>0.05)。绝经后组患者的恶性肿瘤检出率为16%,高于围绝经期组的2%,差异有统计学意义(P<0.05)。结论在围绝经期及绝经后阴道出血患者的诊断中,炎症、非典型性增生、复杂性增生及功能性子宫出血的检出率相当,但绝经后患者的恶性肿瘤检出率更高,提示随着患者年龄增长,癌变的概率也会相应增加。
Objective To investigate the application value of diagnostic curettage in the diagnosis of perimenopausal and postmenopausal vaginal bleeding.Methods A total of 100 patients with vaginal bleeding admitted to our hospital were selected as the research objects.According to the actual conditions of the patients,they were divided into two groups,perimenopausal group(50 cases)and postmenopausal group(50 cases).All patients were required to undergo diagnostic curettage and the results were analyzed.Results In the perimenopausal group,10 cases(20%)of inflammation,18 cases(36%)of atypical hyperplasia,16 cases(32%)of functional uterine bleeding,19 cases(38%)of complex hyperplasia were detected;in the postmenopausal group,12 cases(24%)of inflammation,19 cases(38%)of atypical hyperplasia,14 cases(28%)of functional uterine bleeding,and 20 cases(40%)of complex hyperplasia were detected;there were no significant differences in the detection rate of various causes between the two groups(P>0.05),The detection rate of malignant tumors in postmenopausal group was 16%,which was higher than 2%in perimenopausal group,and the difference was statistically significant(P<0.05).Conclusion In the diagnosis of perimenopausal and postmenopausal patients with vaginal bleeding,the detection rate of inflammation,atypical hyperplasia,complex hyperplasia and functional uterine bleeding is similar,but the detection rate of malignant tumors in postmenopausal patients is higher,suggesting that the probability of cancer will increase with the age of patients.
