血清同型半胱氨酸水平与复发性流产发生的相关性分析 被引量:1

摘要 目的:分析复发性流产和血清同型半胱氨酸间相关性,探讨引起复发性流产患者血清同型半胱氨酸水平上升的主要诱因。方法:纳入2021年1月-2022年1月韶关市妇幼保健院60例非妊娠复发性流产患者(试验组),按照同型半胱氨酸水平划分为正常水平组(n=46)与高水平组(n=14);选择同期60例正常参与产前检查妊娠妇女作为妊娠对照组,60例未妊娠健康体检妇女作为未孕对照组。所有研究对象均检测同型半胱氨酸水平,试验组同时检测维生素B_(12)、叶酸检测。分析三组同型半胱氨酸水平差异,试验组内不同组患者维生素B_(12)、叶酸水平差异,试验组流产次数与同型半胱氨酸水平的相关性。结果:试验组同型半胱氨酸水平高于妊娠对照组、未孕对照组,妊娠对照组同型半胱氨酸水平低于未孕对照组,差异均有统计学意义(P<0.05);三组同型半胱氨酸指标比较,差异有统计学意义(P<0.05)。高水平组维生素B_(12)下降率高于正常水平组(P<0.05);高水平组叶酸下降率高于正常水平组,差异有统计学意义(P<0.05)。其中流产2次同型半胱氨酸水平与流产3~4次比较,差异无统计学意义(P>0.05);流产4次以上同型半胱氨酸水平与流产2次及流产3~4次比较,差异均有统计学意义(P<0.05);不同流产次数同型半胱氨酸水平比较,差异有统计学意义(P<0.05)。采用Spearman秩相关分析显示,试验组流产次数与同型半胱氨酸呈正相关(r_(s)=0.419,P<0.05)。结论:复发性流产与同型半胱氨酸水平上升关系密切,而叶酸丢失为高同型半胱氨酸血症核心诱因,必须早期做好有效防治工作,预防复发性流产风险。 Objective:To analyze the correlation between recurrent abortion and serum homocysteine level,and to explore the main inducement of increasing serum homocysteine level in patients with recurrent abortion.Method:From January 2021 to January 2022,60 patients with non-pregnancy recurrent abortion(the experimental group)in Shaoguan Maternal and Child Health Hospital were included,they were divided into normal level group(n=46)and high level group(n=14)according to homocysteine level;60 pregnant pregnancy women were selected as pregnancy control group,and 60 women without pregnant healthy examination were selected as non-pregnant control group. Homocysteine levels of all subjects were detected, vitamin B_(12) and folic acid in the experimental group were detected. The differences of homocysteine level among three groups, the differences of vitamin B_(12) and folic acid level among different groups in the experimental group, and the correlation between the number of abortions and homocysteine level in the experimental group were analyzed. Result: The homocysteine level in the experimental group was higher than those of pregnant control group and non-pregnant control group, and the homocysteine level of pregnant control group was lower than that of non-pregnant control group (P<0.05). And the level of homocysteine among three groups was significantly different (P<0.05). The decrease rate of vitamin B_(12) of high level group was higher than that of normal level group (P<0.05), the decrease rate of folic acid in high level group was higher than that in normal level group, and the difference was statistically significant (P<0.05). There was no significant difference in homocysteine level between 2 abortions and 3-4 abortions (P<0.05);there were significant differences in homocysteine level between >4 abortions and 2 abortions, 3-4 abortions (P>0.05);and the levels of homocysteine in different abortion times was significantly different (P<0.05).
