摘要
目的:探讨彩色多普勒超声在颈椎病椎动脉供血不足诊断中的应用价值。方法:纳入2021年1-10月广州市东升医院接诊的40例颈椎病椎动脉供血不足患者作为观察组,纳入2021年1-10月广州市东升医院体检中心40例健康体检者作为对照组。所有入选者均接受彩色多普勒超声检查,比较两组椎动脉血流参数检测结果的差异,并分析观察组椎动脉走行异常情况的超声检查结果。结果:观察组椎动脉左侧内径(Dl)、椎动脉右侧内径(Dr)、舒张末期血流速度(EVD)、收缩期峰值血流速度(PSV)、平均血流速度(Vm)、血流量(Q)均低于对照组,阻力指数(RI)、搏动指数(PI)均高于对照组,差异均有统计学意义(P<0.05);观察组多为单侧椎动脉内径偏细,其次为单侧椎动脉走行扭曲,所有走行异常均可在彩色多普勒超声图像上显示。结论:彩色多普勒超声能为颈椎病椎动脉供血不足诊断提供可靠且详细的血流参数,临床应用价值较高。
Objective:To investigate the value of color Doppler ultrasound in the diagnosis of vertebral artery insufficiency in cervical spondylosis.Method:A total of 40 patients with vertebral artery insufficiency in cervical spondylosis from January to October 2021 were included as the observation group,a total of 40 healthy physical examinations in the physical examination center of our hospital from January to October 2021 were included as the control group.All inductees underwent color Doppler ultrasound,the difference of vertebral artery flow parameters between the two inductee groups were compared,and the ultrasound results of abnormal shape of vertebral artery in the observation group was analyzed.Result:The left internal diameter (Dl),right internal diameter (Dr),end-diastolic blood velocity (EVD),systolic peak blood velocity (PSV),mean blood velocity (Vm) and blood flow (Q) in the observation group were lower than those in the control group,resistance index (RI) and pulsatility index (PI) were higher than those of the control group,the differences were statistically significant (P<0.05).The observation group had thin unilateral internal diameter,followed by unilateral vertebral artery distortion,all walking abnormalities could be displayed on color Doppler ultrasound images.Conclusion:Color Doppler ultrasound can provide reliable and detailed blood flow parameters for the diagnosis of vertebral artery insufficiency in cervical spondylosis,which has a high clinical application value.
