超声引导下细针穿刺细胞学检查与超声弹性成像技术在甲状腺结节诊断中的临床价值 被引量:16

摘要 目的 探讨超声引导下细针穿刺细胞学检查与超声弹性成像技术在甲状腺结节诊断中的临床价值。方法 选择2019年4月至2021年3月在河南省直第三人民医院进行检查的135例甲状腺结节患者(共计192个结节)作为研究对象,后进行甲状腺超声弹性成像检查和超声引导下细针穿刺活检,以手术病理作为确诊结果,评价2种检查的临床诊断价值。结果 所有研究对象手术病理结果显示,良恶性结节分别125个和67个;超声引导下细针穿刺活检对甲状腺结节的诊断准确率(185个,96.35%)高于超声弹性成像检查(175个,91.15%)(P<0.05);超声引导下细针穿刺活检对甲状腺恶性结节诊断的灵敏度、特异度、约登指数、阳性预测值和阴性预测值(分别为95.52%、96.80%、0.923、94.12%和97.58%)均高于超声弹性成像检查(分别为88.06%、92.80%、0.809、86.76%和93.55%)。结论 超声引导下细针穿刺细胞学检查能够有效提高甲状腺良恶性结节的诊准确率,临床应用价值较高,可为甲状腺疾病治疗方案的制定提供依据。 Objective To explore the clinical value of ultrasound-guided fine-needle aspiration cytology and ultrasound elastography in the diagnosis of thyroid nodules.Methods A total of 135 patients with thyroid nodules(192 nodules) in the Third People’s Hospital of Henan province from April 2019 to March 2021 were selected as the research objects.All subjects were underwent ultrasound elastography and ultrasound-guided fine-needle aspiration cytology,with surgical pathology as the diagnosis result.The clinical diagnostic value of the two examinations was evaluated.Results The surgical and pathological results of all subjects showed that there were 125 and 67 benign and malignant nodules respectively.The diagnostic accuracy of the ultrasound-guided fine-needle aspiration cytology for thyroid nodules(185 nodules,96.35%) was higher than that of ultrasound elastography(175 nodules,91.15%)(P<0.05).The sensitivity,specificity,Youden index,positive predictive value and negative predictive value of the ultrasound-guided fine-needle aspiration cytology for the diagnosis of breast malignant thyroid nodules(95.52%,96.80%,0.923,94.12% and 97.58%,respectively) were higher than ultrasound elastography(88.06%,92.80%,0.809,86.76% and 93.55%,respectively).Conclusion Ultrasound-guided fine-needle aspiration cytology could effectively improve the diagnostic accuracy of benign and malignant thyroid nodules,have high clinical application value and provide the basis for the treatment of thyroid disease.
