摘要
目的探析在肺周围型病变诊断中应用超声支气管镜引导下经支气管肺活检术(EBUS-TBLB)的临床价值。方法2020年1月至2021年10月选取揭阳市人民医院呼吸与危重症医学科收治的80例肺周围型病变患者作为研究对象,将入选的患者按照随机数表法分为观察组和对照组,每组各40例,观察组患者应用EBUS-TBLB行经支气管肺活检,对照组患者采用在CT引导下行经皮肺穿刺术(CT-PNB)获取肺组织标本,然后比较两组患者在病理确诊阳性率、并发症发生率、费用方面的差异。结果观察组40例患者中34例成功获得标本且明确诊断,诊断阳性率为85.00%;对照组40例患者中25例成功获得标本且明确诊断,诊断阳性率为62.50%,差异有统计学意义(P<0.05);观察组并发症总发生率为7.50%;对照组并发症总发生率为25.00%,观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。观察组检查费用为(1670±125)元,比对照组的(1702±138)元略少,差异无统计学意义(P>0.05)。40例经超声小探头引导下经支气管镜肺活检患者中,34例患者成功检出肺周围型病变,其余6例患者未能检出。病灶直径>2 cm的阳性检出率为92.00%(23/25)高于病灶直径≤2 cm的73.33%(11/15)(χ^(2)=0.961,P=0.327),探头被病灶包绕的阳性检出率为92.31%(24/26),高于不被病灶包绕的71.43%(10/14)(χ^(2)=1.167,P=0.280)。结论在肺周围型病变诊断中,EBUS-TBLB的应用可以使诊断阳性率提高,而且还可以减少并发症的发生率,具备良好的安全性,诊断价值显著。
Objective To investigate the clinical value of endobronchial ultrasound-guided transbronchial lung biopsy(EBUS-TBLB)in the diagnosis of peripheral pulmonary lesions.Methods A total of 80 patients with peripheral pulmonary lesions admitted to the Department of Respiratory and Critical Care Medicine of Jieyang People’s Hospital from January 2020 to October 2021 were selected as study subjects,and were divided into an observation group and a control group according to the random number table method,with 40 patients in each group.Lung tissue specimens were obtained from patients in the observation group by EBUSTBLB,and from patients in the control group by computed tomography(CT)-guided percutaneous needle biopsy(CT-PNB).The differences in terms of the positive rate of pathological diagnosis,incidence of complications,and cost of examination were compared between the two groups.Results In the observation group,34 cases were diagnosed with a positive rate of 85.00%.Of the 40 patients in the control group,25 were confirmed with a positive rate of 62.50%,with statistically significant difference(P<0.05).The total incidence of complications in the observation group(7.50%)was lower than that in the control group(25.00%),with statistically significant difference(P<0.05).The cost of e x a m i n a t i o n i n t h e o b s e r v a t i o n g r o u p([1670±125]y u a n)w a s slightly less than that in the control group([1702±138]yuan),with no statistically significant difference(P>0.05).Among 40 patients who underwent EBUS-TBLB,peripheral pulmonary lesions were detected in 34 patients,and not detected in the remaining 6 patients.The positive detection rate was 92.00%(23/25)for patients with lesions>2 cm in diameter,higher than 73.33%(11/15)for patients with lesions≤2 cm in diameter(χ^(2)=0.961,P=0.327),and the positive detection rate was 92.31%(24/26)for patients with the probe encircled by lesions,higher than 71.43%(10/14)for patients with the probe not encircled by lesions(χ^(2)=1.167,P=0.280).Conclusion The EBUS-TBLB c
