摘要
目的 观察宁心贴穴位贴敷联合西药治疗心脾两虚型室性早搏的临床疗效。方法 将70例心脾两虚型室性早搏患者随机分为治疗组和对照组,每组35例,对照组予盐酸普罗帕酮治疗,治疗组在对照组治疗措施的基础上加用宁心贴外敷双侧内关穴,两组疗程均为4周。观察临床疗效,比较中医证候积分、24 h动态心电图指标、心律失常危险分层的变化情况。结果 (1)治疗组、对照组中医证候总有效率分别为88.57%和60.00%,室性早搏总有效率分别为91.43%和80.00%;两组中医证候疗效、室性早搏疗效比较,治疗组优于对照组(P<0.05,P<0.01)。(2)治疗前后组内比较,两组中医证候积分、室性早搏次数均减少(P<0.05);组间治疗后比较,治疗组中医证候积分、室性早搏次数少于对照组(P<0.05,P<0.01)。(3)心率变异性指标[总体标准差(SDNN),平均标准差(SDANN),差值均方根(RMSSD),超过50 ms百分比(pNN50)]变化情况:治疗前后组内比较,两组SDNN、SDANN、RMSSD、pNN50值升高(P<0.05);组间治疗后比较,治疗组SDNN、SDANN、RMSSD、pNN50值高于对照组(P<0.05,P<0.01)。(4)治疗后,两组心律失常危险分层均得到改善(P<0.05),且治疗组的改善情况优于对照组(P<0.05)。(5)治疗期间,两组均未见明显的不良反应。结论 宁心贴穴位贴敷联合西药治疗心脾两虚型室性早搏疗效满意,与单纯使用西药相比,加用宁心贴穴位贴敷能更好地改善患者的临床症状和预后,且安全性良好。
Objective ventricular extrasystole with syndrome of deficiency of heart and spleen.Methods deficiency of heart and spleen were randomly divided into the control group and treatment group,35 patients in each group. The control group was treated with propafenone hydrochloride,and the treatment group was treated with Ningxin Paste by acupoint application at bilateral Neiguan acupoints.The course of treatment in both groups was 4 weeks. The clinical efficacy was observed,and the changes of Chinese medical syndrome score,24 h dynamic electrocardiogram(ECG)indexes and arrhythmia risk stratification were compared.Results syndrome in the treatment group and the control group were 88.57% and 60.00% respectively,and the total effective rates of ventricular extrasystole were 91.43% and 80.00% respectively. The efficacy of the treatment group was better than that of the control group(P<0.05,P<0.01).(2)Compared ,and the number of ventricular extrasystole were decreased in both groups after treatment(P<0.05). After treatment, the score of Chinese medical syndrome and the number of ventricular extrasystole in the treatment group were less than those in the control group(P<0.05, P<0.01).(3) Changes of heart rate variability indexes[overall standard deviation(SDNN),average standard deviation(SDANN),root mean square difference(RMSSD),percentage of exceeding 50 ms(pNN50)]:Compared with those before treatment,the values of SDNN,SDANN,RMSSD and pNN50 were increased in both groups(P<0.05). After treatment,the values of SDNN,SDANN,RMSSD and pNN50 in the treatment group were higher than those in the control group(P<0.05,P<0.01).(4)After the treatment,the risk stratification of arrhythmia was improved in both groups(P<0.05),and the improvement in the treatment group was better than that in the control group(P<0.05).(5)During the treatment,there were no obvious adverse reactions in both groups.ConclusionsAcupoint application of Ningxin Paste combined with western medicine shows good efficacy in the treatment of ventricular extrasystole wi
