摘要
腹膜纤维化(PF)是导致腹膜透析(PD)患者腹膜超滤功能衰竭的主要原因,腹膜间皮细胞(PMC)发生上皮细胞-间充质转分化(EMT)是PF的起始、可逆的生理过程,在PF的发生、发展中起着关键作用。PMC内质网应激(ERS)可能通过IRE1α/XBP1通路及其诱导的自噬参与间皮细胞EMT进程。脾气亏虚是PD相关的腹膜早期病变的主要病机,本文主要探讨了PD相关性PF PMC脾气亏虚的分子机制。PMC ERS及其诱导的自噬与中医“脾气亏虚”存在密切相关性,可能是脾气亏虚的微观机制。以健脾益气法防治PD相关性腹膜损伤病变有较好的疗效。
Peritoneal fibrosis(PF)is the main cause of peritoneal ultrafiltration failure in peritoneal dialysis(PD)patients.Epithelial-to-mesenchymal transition(EMT)of peritoneal mesothelial cells(PMC)is an initial and reversible physiological process of PF,which plays a key role in the occurrence and development of PF.Endoplasmic reticulum stress(ERS)in PMC may be involved in the EMT process through IRE1α/XBP1 pathway and its induced autophagy.Spleen qi deficiency is the main pathogenesis of PD-related early peritoneal lesions.This paper mainly discusses the molecular mechanism of spleen qi deficiency in PD-related PF PMC.ERS in PMC and its induced autophagy are closely related to spleen qi deficiency in traditional Chinese medicine,which may be the microscopic mechanism of spleen qi deficiency.The method of invigorating spleen and replenishing qi is effective in preventing and treating PD-related peritoneal injury.
