摘要
目的:探讨超声引导下经皮子宫肌瘤微波消融术后短期临床疗效。方法:回顾性分析2018年6月—2021年12月在汕头大学医学院第二附属医院行超声引导下经皮微波消融治疗的75例子宫肌瘤患者的临床资料。按照术前磁共振显示的子宫肌瘤位置分为近黏膜组(36例)和远黏膜组(39例),比较两组术后3个月子宫肌瘤体积缩小率和临床症状改善得分。结果:微波消融治疗后3个月,子宫肌瘤体积缩小率为(61.2±20.1)%,近黏膜组子宫肌瘤体积缩小率大于远黏膜组[分别为(68.0±18.7)%、(55.0±19.6)%],近黏膜组临床症状改善得分优于远黏膜组[症状严重程度改善得分分别为14.00 (9.50,17.25)、7.00 (5.75,11.50),健康相关的生活质量改善得分分别为42.00 (32.00,55.50)、34.00 (24.75,43.50)],差异均有统计学意义(P值均<0.05)。结论:超声引导经皮微波消融治疗子宫肌瘤的短期疗效确切,靠近黏膜的子宫肌瘤术后短期临床疗效优于远离黏膜的子宫肌瘤。
Objective: To investigate the short-term clinical effect of ultrasound-guided percutaneous microwave ablation of hysteromyoma. Methods: The clinical data of 75 patients with hysteromyoma who underwent ultrasound-guided percutaneous microwave ablation in the Second Affiliated Hospital of Shantou University Medical College from June 2018 to December 2021 were retrospectively analyzed. According to the location of hysteromyoma displayed by preoperative magnetic resonance imaging, they were divided into a near-mucosal group(36 cases) and a far-mucosal group(39 cases). The volume reduction rate and clinical symptom improvement score of hysteromyoma were compared between the two groups at 3 months after operation. Results:Three months after microwave ablation, the volume reduction rate of hysteromyoma was(61.2±20.1)%, and the volume reduction rate of hysteromyoma in the near-mucosal group was greater than that in the far-mucosal group[respectively(68.0±18.7)%,(55.0±19.6)%], the clinical symptom improvement score of the near-mucosal group was better than that of the far-mucosal group [symptom severity improvement scores of 14.00(9.50, 17.25) and7.00(5.75, 11.50) and health-related quality of life improvement scores of 42.00(32.00, 55.50) and 34.00(24.75,43.50), respectively], and the differences were statistically significant(all P<0.05). Conclusion: The short-term efficacy of ultrasound-guided percutaneous microwave ablation in the treatment of hysteromyoma is definite, and the short-term clinical efficacy of hysteromyoma close to the mucosa is better than that of hysteromyoma far away from the mucosa.
