摘要
目的比较不同手术入路治疗后踝骨折的临床疗效。方法对2018年1月-2020年10月收治的84例后踝骨折患者,以随机数字表法分为两组,每组各42例。A组以改良后内侧入路进行手术,B组采用外侧入路手术。比较两组临床疗效、手术一般情况、踝关节活动度及并发症发生情况。结果A组总有效率为83.33%,高于B组的61.90%(P<0.05)。A组患者手术时间、术中出血量及愈合时间均较B组更短(P<0.05)。术后两组患者跖屈、背屈角度均大于术前,且A组均大于B组;术后两组患者外翻、内翻角度均小于术前,且A组均小于B组(P<0.05)。A组共出现2例(4.76%)并发症,B组出现3例(7.14%)不良反应,差异无统计学意义(P>0.05)。结论后踝骨折患者行改良后内侧入路手术较外侧入路手术可有效提高术后踝关节功能优良率,减少术中出血量,缩短手术时间及愈合时间,改善术后踝关节活动度。
Objective To compare the clinical effects of different approaches for posterior malleolus fractures.Methods A total of 84 posterior malleolus fracture patients admitted to our hospital from January 2018 to October 2020 were selected and divided into 2 groups by random number table,with 42 cases in each group.Group A was operated with a modified posterior medial approach,and group B was operated with a lateral approach.The clinical efficacy,general operation conditions,ankle range of motion and complications of the two groups were compared.Results The total effective rate of group A was 83.33%,which was higher than 61.90%of group B(P<0.05).The operation time,intraoperative blood loss and healing time in group A were shorter than those in group B(P<0.05).The angles of plantar flexion and dorsiflexion of the two groups of patients after surgery were greater than those of the preoperative group,and the angles of group A were greater than those of group B;the angles of valgus and varus of the two groups of patients after surgery were both smaller than those of preoperative group,and the angles of group A were smaller than those of group B(P<0.05).There were 2 cases(4.76%)of complications in group A and 3 cases(7.14%)of adverse reactions in group B.There was no significant difference between the two groups(P>0.05).Conclusion Compared with lateral approach surgery,patients with posterior malleolus fractures undergoing modified medial approach surgery can effectively improve the postoperative ankle function excellent rate,shorten the operation time,intraoperative blood loss and healing time,and improve postoperative ankle mobility.
