手动和电动牙刷控制口腔卫生的临床随机对照试验 被引量:2

A clinical randomized controlled trial of manual and powered toothbrushes for oral hygiene control

摘要 目的 比较手动牙刷和电动牙刷控制口腔卫生和牙龈炎症的效果。方法 试验共纳入51名受试者,随机分为手动牙刷组和电动牙刷组,观察期为6个月,被访问5次,分别为基线、5天、6周、3个月和6个月。每次均记录改良牙龈指数(modified gingival index, MGI)、牙龈出血指数(gingival bleeding index, GBI)和改良菌斑指数(modified plaque index, MPI),其中,第5天时,仅记录改良菌斑指数(MPI)。本研究采用非参数检验比较各访视的组间差异,采用广义估计方程比较组间的趋势变化差异。结果 在第5天时,与手动牙刷相比,电动牙刷控制牙菌斑的效果更好(P<0.05),在6个月内,电动牙刷控制牙菌斑的效果与手动牙刷相当。在6个月内,两组的MGI绝对减少量随时间均呈上升趋势,电动牙刷组MGI绝对减少量的上升趋势比手动牙刷组大(P<0.05);两组GBI绝对减少量均随时间呈上升趋势,且电动牙刷比手动牙刷组有更少的牙龈出血,但组间差异无统计学意义(P>0.05)。结论 本研究表明电动牙刷在1周内减少牙菌斑的效果优于手动牙刷。虽然两种牙刷都能不同程度地减少牙龈炎症,但电动牙刷缓解牙龈炎症的速度更快。为保持良好的口腔卫生,掌握正确的刷牙方法和强化口腔卫生宣教比牙刷的类型更为重要。 Objective To compare the effectiveness of the manual and powered toothbrushes in controlling oral hygiene and gingival inflammation. Methods A total of 51 subjects were enrolled and randomly divided into a manual toothbrush group and a powered toothbrush group. The observation period was 6 months, and subjects were interviewed 5 times at baseline, 5 days, 6 weeks, 3 months and 6 months, respectively. Modified Gingival Index(MGI), Gingival Bleeding Index(GBI) and Modified Plaque Index(MPI) were recorded each time except that only the Modified Plaque Index(MPI) was recorded at the first follow-up(at 5 days). In the study, the non-parametric test was used to compare the differences between the two groups, and Analysis were performed to compare the variation of trend between the two groups with generalized estimation equation model. Results At 5 days, the powered toothbrush group performed better in reduction of plaque than manual toothbrush group(P<0.05). Within6 months, the plaque control effect in the powered toothbrush group was comparable to that in the manual toothbrush group. In 6 months, the absolute reduction of MGI showed an increasing trend in both groups. The decrease of MGI in the powered toothbrush group showed a sharper upward trend than that in the manual toothbrush group(P<0.05). In both groups, the absolute reduction of GBI showed a rising trend over time, the powered toothbrush group had less gingival bleeding than manual toothbrush group, but there was no significant difference between the groups(P >0.05). Conclusion This study showed that powered toothbrushes are more effective in reducing plaque than manual toothbrush within one week. Both manual toothbrushes and powered toothbrushes could reduce gingival inflammation in varying degrees, but powered toothbrushes reduced gingival inflammation faster. To maintain good oral hygiene, it’s more important to master the correct method of brushing teeth and strengthen oral hygiene instruction than the type of toothbrush.
