摘要
目的回顾分析45例小肠非特异性溃疡患者使用甲硝唑联合复方谷氨酰胺治疗前后的临床资料,探讨联合用药的疗效及安全性,并查阅复习相关文献,分析联合用药的可能作用机制。方法选取2018年2月~2021年6月安徽省宿州市永安医院消化内科收治的45例小肠非特异性溃疡患者作为回顾分析研究对象,分析统计其治疗前后的临床资料。结果(1)疗效方面:①治疗后肠镜检查总有效率为91.11%;②治疗后患者腹部症状改善明显,症状积分显著下降(P<0.05);③治疗后患者血清学炎性因子TNF-α、IL-6、IL-8表达水平较治疗前明显降低(P<0.05);④治疗后患者C反应蛋白(CRP)水平、红细胞沉降率(ESR)显著低于治疗前(P<0.05);(2)安全性方面:①治疗后患者血常规及肝肾功能相关参考指标同治疗前比较差异无统计学意义(P>0.05);②治疗期间患者胃肠系统、神经系统无严重或不可控不良反应发生。结论(1)甲硝唑联合复方谷氨酰胺对小肠非特异性溃疡总体疗效较好、安全性高;(2)联合用药的作用机制可能为两药均具有减少炎性因子产生、促进损伤肠黏膜恢复的作用。
Objective The clinical data of 45 patients with small intestinal nonspecific ulcer before and after treatment with metronidazole combined with compound glutamine were retrospectively analyzed,the efficacy and safety of the combination were discussed,and the relevant literature was reviewed to analyze the possible mechanism of the combination.Methods 45 patients with small intestinal nonspecific ulcer treated in the Department of Gastroenterology,Yong'an hospital,Suzhou City,Anhui Province from February 2018 to June 2021 were selected as the subjects of retrospective analysis,and the clinical data before and after treatment were analyzed and counted.Results(1)Curative effect:①the total effective rate of enteroscopy after treatment was 91.11%;②After treatment,the patients'abdominal symptoms improved significantly,and the symptom score decreased significantly(P<0.05);③Serum inflammatory factor TNF in patients after treatment-α,the expression levels of IL-6 and IL-8 were significantly lower than those before treatment(P<0.05);④After treatment,the level of C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)were significantly lower than those before treatment(P<0.05);(2)Safety:①there was no significant difference in blood routine and liver and kidney function related reference indexes between patients after treatment and before treatment(P>0.05);②There were no serious or uncontrollable adverse reactions in the gastrointestinal system and nervous system during the treatment.Conclusion(1)Metronidazole combined with compound glutamine has good overall efficacy and high safety in the treatment of small intestinal nonspecific ulcer;(2)The mechanism of combined medication may be that both drugs can reduce the production of inflammatory factors and promote the recovery of damaged intestinal mucosa.
