摘要
目的研究后牙单冠种植修复中应用螺丝固位一体化基台冠的效果及优势。方法100例需进行后牙单冠种植修复的患者,完成种植手术3~6个月后均进行螺丝固位一体化基台进行修复。分析患者改良出血指数(mSBI)分布情况、种植体存活率、不同平台深度种植体出血情况及术后1、2年骨吸收量;比较修复前后患者主观感受评分。结果100例(100颗牙)患者种植体支持在后牙单冠修复中,上颌59颗,下颌41颗。mSBI记录分别为0级52例(52.00%)、1级33例(33.00%)、2级15例(15.00%)、3级0例;所有患者基台都未出现松动,也无修复体松动,种植体存活率为100.00%(100/100)。平台深度1级患者20例,其中牙周探诊出血阴性[BOP(-)]14例,牙周探诊出血阳性[BOP(+)]6例;2级患者61例,其中BOP(-)33例,BOP(+)28例;3级患者19例,其中BOP(-)5例,BOP(+)14例。修复后,患者对固定功能评分(8.98±0.3)分、言语功能评分(7.70±1.56)分、咀嚼能力评分(7.55±0.22)分、舒适度评分(8.98±1.00)分、美观性评分(7.23±1.04)分均高于修复前的(8.30±0.23)、(7.20±0.52)、(7.20±1.06)、(8.45±0.24)、(6.72±0.25)分,差异具有统计学意义(P<0.05)。术后1年,患者骨吸收量为(0.44±0.25)mm;术后2年骨吸收量为(0.544±0.30)mm。结论在后牙单冠种植修复中使用螺丝固位一体化基台冠固位力良好,且无粘接剂残留,修复成功率较高,值得在临床上大范围的推广以及应用。
Objective To study the effect and advantages of screw-retained integrated abutment crown in posterior single-crown implant restorations.Methods 100 patients requiring posterior single-crown implant restorations were restored with screw-retained integrated abutments at 3-6 months after completion of the implant surgery.The patients'modified bleeding index(mSBI)distribution,implant survival rate,implant bleeding at different platform depths and bone resorption at 1 and 2 years postoperatively were analyzed;the patients'subjective perception scores before and after restoration were compared.Results 100 patients(100 teeth)had implant-supported single-crown restorations in the posterior teeth,59 teeth of which were in the maxillary and 41 teeth of which were in the mandible.The mSBI records were grade 0 in 52 cases(52.00%),grade 1 in 33 cases(33.00%),grade 2 in 15 cases(15.00%),and grade 3 in 0 cases;no abutment loosening or restoration loosening was observed in all patients,and the implant survival rate was 100.00%(100/100).20 patients with grade 1 platform depth,of which 14 cases were negative for bleeding on probing[BOP(-)]and 6 cases were positive for bleeding on probing[BOP(+)];61 patients with grade 2 platform depth,of whom 33 cases were BOP(-)and 28 cases were BOP(+);19 patients with grade 3 platform depth,of whom 5 cases were BOP(-)and 14 cases were BOP(+).After restoration,the fixation function score(8.98±0.3)points,speech function score(7.70±1.56)points,masticatory ability score(7.55±0.22)points,comfort score(8.98±1.00)points,and aesthetic score(7.23±1.04)points of patients were higher than(8.30±0.23),(7.20±0.52),(7.20±1.06),(8.45±0.24),and(6.72±0.25)points before restoration,and the differences were statistically significant(P<0.05).At 1 year postoperatively,the patient's bone resorption was(0.44±0.25)mm;at 2 years postoperatively,the bone resorption was(0.544±0.30)mm.Conclusion Screw-retained integrated abutment crown has good retention force and no adhesive residue in posterior single-crown im
