单个种植牙修复后冠松动率的临床特点分析

摘要 目的探究分析单个种植牙修复后冠松动率的临床特点。方法将我院于2019年12月至2020年12月口腔科收治的96例进行单冠种植术治疗的牙修复患者纳入本组研究,共计138个单牙种植体,收集整理患者的所有基本临床资料,对其进行单个种植牙修复治疗,治疗后随访2年,观察在随访期间的牙冠松动情况,总结其临床特点,分析引起牙冠松动的因素。结果①在138个单牙种植体中,MIS系统71个,其松动率为1.41%(1/71);DIO系统26个,其松动率为3.85%(1/26);CDIC系统22个,其松动率为13.64%(3/22);NOBEL系统19个,未有冠松动的情况的单牙种植体出现,其松动率为0%;②在138个单牙种植体中,共有63个前牙种植牙,其松动率为1.59%(1/63);有75个后牙种植牙,其松动率为5.33%(4/75);③单个种植牙修复治疗后,相较于未发生冠松动前,冠松动后,患者的VAS疼痛评分、PSQI睡眠质量评分均更高,生活质量评分更低,数据经SPSS 22.0统计学软件分析对比均显示(P<0.05)。结论在口腔科临床中,对实施单个种植牙修复的患者而言,相较于MIS系统、DIO系统、NOBEL等种植体系统,CDIC系统更易出现冠松动的情况,而在所有单牙种植体中,相较于前牙种植,后牙种植更易出现冠松动的情况。经观察发现,这可能与后牙的咀嚼力相关,并且出现冠松动后,会引起患者牙疼,影响其睡眠质量和生活质量,需要及时采取有效干预措施,提高治疗预后。 Objective To explore and analyze the clinical characteristics of crown loosening rate after single implant restoration.Methods From December 2019 to December 2020,96 patients with single crown implant in our hospital were included in this study.A total of 138 single dental implants were collected.All the basic clinical data of the patients were collected and treated with single implant.The patients were followed up for 2 years after treatment to observe the crown loosening during the follow-up period and summarize the clinical characteristics,Objective to analyze the causes of crown loosening.Results①Among 138 single-tooth implants,71 were MIS systems,with a loosening rate of 1.41%(1/71).Twenty-six DIO systems,with a loosening rate of 3.85%(1/26).Twenty-two CDIC systems,with a loosening rate of 13.64%(3/22).In 19 NOBEL systems,single-tooth implants without crown loosening appeared,and the loosening rate was 0%.②Among 138 single dental implants,63 anterior dental implants had a loosening rate of 1.59%(1/63),and 75 posterior dental implants had a loosening rate of 5.33%(4/75).③After the single implant restoration,compared with the patients without crown loosening and after crown loosening,VAS pain score and PSQI sleep quality score were higher,and the quality of life score was lower.The data were analyzed and compared with SPSS 22.0 statistical software(P<0.05).Conclusion Compared with MIS system,DIO system and noble system,CDIC system is more prone to crown loosening in dental clinic.Compared with anterior implant,posterior implant is more prone to crown loosening in all single dental implants.After observation,it is found that this may be related to the masticatory force of posterior teeth,and after crown loosening,it will cause toothache and affect the sleep quality and quality of life of patients.It is necessary to take effective intervention measures in time to improve the treatment prognosis.
