摘要
目的探讨超声评估下喂养策略在重症监护室(ICU)有创通气患者肠内营养(EN)支持中的应用效果。方法选取我院重症监护室(ICU)有创通气重症患者146例,随机分为两组各73例。两组均行EN支持,对照组以注射器回抽法监测胃残留量(GRV)指导喂养,研究组以超声胃窦单切面法测定GRV、胃窦运动指数(MI),据此指导喂养,均干预1周。比较两组的营养参数水平及EN达标时间、恢复时间、30 d病死率、EN并发症情况。结果干预后,研究组营养参数水平优于对照组,EN达标时间、呼吸机使用时间及ICU入住时间短于对照组,且腹胀、腹泻、EN中断、呼吸机相关性肺炎发生率低于对照组(P<0.05)。结论超声评估下喂养策略可改善ICU有创通气重症患者营养状况,促进患者恢复,减少EN并发症。
Objective To explore the application effect of feeding strategy under ultrasound assessment in enteral nutrition(EN)support for patients with invasive ventilation in intensive care unit(ICU).Methods 146 patients with invasive ventilation in ICU in our hospital were selected and randomly divided into study group and control group,with 73 cases in each group.Both groups were given EN.The control group was fed under the guidance of gastric residual volume(GRV)monitored by syringe withdrawal method,and the study group was fed under the guidance of GRV and antral motility index(MI)measured by ultrasonic antrum single section method.All were intervened for 1 week.The nutritional parameter level,EN compliance time,recovery time,30-day mortality rate and EN complications were compared between the two groups.Results After intervention,the levels of nutritional parameters in the study group were better than those in the control group,the EN compliance time,ventilator use time and ICU stay time were shorter than those in the control group,and the incidences of abdominal distension,diarrhea,EN interruption,and ventilator-associated pneumonia were lower than those in the control group(P<0.05).Conclusions Feeding strategy under ultrasound assessment can improve the nutritional status of ICU patients with invasive ventilation,promote patients'recovery,and reduce the EN complications.
