肺腺癌患者血清B7-H3、NKG2D的表达及其与化疗耐药性的关系

摘要 目的观察肺腺癌患者血清共刺激分子(B7-H3)、自然杀伤细胞活化性受体2D(NKG2D)的表达,分析二者与患者接受培美曲塞联合顺铂(简称PP)化疗后耐药性的关系。方法选取许昌市人民医院2018年3月至2019年3月接收的95例肺腺癌患者(均接受PP化疗)作为研究对象。化疗4个周期后随访1个月,根据化疗效果分为耐药组和敏感组。比较两组患者基线资料和血清B7-H3、NKG2D水平,探讨血清B7-H3、NKG2D水平与肺腺癌患者PP化疗耐药的关系。结果肺腺癌患者经PP治疗后耐药率为58.95%。耐药组血清B7-H3水平高于敏感组,NKG2D水平低于敏感组(P<0.05)。logistic回归结果显示,血清B7-H3、NKG2D水平与肺腺癌患者PP化疗耐药有关(P<0.05);绘制受试者工作特征(ROC)曲线,化疗前血清B7-H3、NKG2D水平预测肺腺癌患者PP化疗耐药的曲线下面积(AUC)分别为0.824、0.772,联合预测为0.853。结论肺腺癌化疗耐药患者血清B7-H3升高、NKG2D降低,且二者均与PP化疗耐药性有关,二者联合预测肺腺癌患者化疗耐药性的价值最高。 Objective To observe the expression of serum B7 homolog(B7-H3)and natural killer cell group 2D(NKG2D)in patients with lung adenocarcinoma,and analyze the relationship between them and drug resistance after receiving pemetrexed and cisplatin(PP).Methods A total of 95 patients with lung adenocarcinoma(all receiving PP chemotherapy)treated in Xuchang people’s Hospital from March 2018 to March 2019 were selected as the study subjects.The patients were followed up for 1 month after 4 cycles of chemotherapy,and were divided into drug-resistant group and sensitive group according to the effect of chemotherapy.To compare the baseline data and serum B7-H3 and NKG2D levels between the two groups,and to explore the relationship between serum B7-H3 and NKG2D levels and PP chemotherapy resistance in patients with lung adenocarcinoma.Results The drug resistance rate of lung adenocarcinoma patients treated with PP was 58.95%.The level of serum B7-H3 in drug-resistant group was higher than that in sensitive group,and the level of NKG2D was lower than that in sensitive group(P<0.05).Logistic regression analysis showed that the levels of serum B7-H3 and NKG2D were related to PP chemotherapy resistance in patients with lung adenocarcinoma(P<0.05).The receiver operating characteristic(ROC)curve of the subjects was drawn.The area under curve(AUC)of serum B7-H3 and NKG2D levels before chemotherapy to predict PP chemotherapy resistance in lung adenocarcinoma patients was 0.824 and 0.772 respectively,and the combined prediction was 0.853.Conclusion Serum B7-H3 increased and NKG2D decreased in patients with chemotherapy resistance of lung adenocarcinoma,and both of them were related to PP chemotherapy resistance.The combination of them had the highest value in predicting chemotherapy resistance of lung adenocarcinoma patients.
