摘要
目的观察新型口服利尿剂托伐普坦对心力衰竭患者,在常规药物治疗的基础上使用的疗效与安全性.方法回顾性收集广元市中心医院药品目录引进托伐普坦期间在院接受托伐普坦治疗的心力衰竭患者48例为托伐普坦组,随机筛选临床诊断为心力衰竭相关病例、接受常规药物治疗并未使用托伐普坦的患者48例作为对照组.通过比较托伐普坦组与对照组患者24h尿量、N末端B型脑钠肽前体(NT-proBNP)、血清钠、血清钾、血肌酐、血尿酸水平进行疗效对比.结果诊疗后托伐普坦组患者的排出尿量显著增多,差异有统计学意义(P<0.05);血清钠、血清钾、肌酐、尿酸、NT-proBNP变化水平,差异均无统计学意义(P>0.05).结论心力衰竭患者使用常规药物诊疗的同时,应用新型口服利尿剂托伐普坦相较常规疗法可更强地增加排尿量,有效消除患者的水钠潴留症状,对血清钠、血清钾、血肌酐、尿酸、NT-proBNP的水平无显著影响.
Objective To observe the efficacy and safety of tolvaptan,a new oral diuretic,on the basis of conventional drug therapy in patients with heart failure.Methods Forty-eight patients with heart failure who received tolvaplan treatment during the introduction of tolvaptan in Guangyuan Central Hospital were selected as tolvaptan group.Forty-eight patients with clinical diagnosis of heart failure and receiving conventional treatment were randomly selected and divided into control group.The efficacy was compared by comparing 24h urine volume,N-terminal B-type brain natriuretic peptide precursor(NT-proBNP)serum sodium,serum potassium,serum creatinine and blood uric acid levels in the two groups.Resul ts Compared with the control group,he 24h urine volume of patients in the tolvaptan group increased significantly after treatment,with statistical significance(P<0.05).There were no significant differences in serum sodium,serum potassium,creatinine,uric acid and NT-proBNP(P>0.05).Conclusion On the basis of conventional treatment of heart failure patients using the new oral diuretic tolvaptan compared with conventional therapy can be stronger to increase the urination dose,ffectively eliminate the symptoms of water and sodium relention of patients,serum sodium,serum potassium,serum creatinine,uric acid,NT-proBNP levels have no significant impact.
