摘要
目的探究加味大承气汤对胃癌根治术患者术后早期肠功能恢复的影响。方法选取铁岭市中心医院胃肠科收治的胃癌根治术后患者72例,随机分为对照组与试验组,各36例。对照组术后予以头孢唑林2.0 g肌肉注射,每日2次,山莨菪碱10 mg肌肉注射,日4次;试验组联用加味大承气汤,日1剂,3 d为1个疗程。治疗结束后比较2组经口进食时间、肛门排气排便时间、肠硬阻发生率、术后住院时间、血浆胃泌素(MOT)、血管活性肠肽(VIP)、生长抑素(SS)、P物质(SP)及不良反应发生率。结果治疗后,与治疗前相比2组MOT、SS、SP升高,VIP降低,且试验组优于对照组(P<0.05);治疗后与对照组相比,试验组经口进食时间、肛门排气排便时间、住院时间缩短,肠梗阻发生率、不良反应发生率较低(P<0.05)。结论加味大承气汤应用于胃癌根治术后患者的治疗中,可促进术后肠道功能恢复,缩短排气、排便恢复时间,有效减少术后并发症,适宜临床应用。
Objective To explore the effect of modified Dachengqi decoction on the early recovery of intestinal function in patients with radical gastrectomy.Methods Seventy-two patients with gastric cancer after radical gastrectomy admitted to Tieling Central Hospital were randomly divided into control group and experimental group,with 36 cases in each group.The control group was given cefazolin 2.0 g intramuscular injection,twice a day,anisodamine 10 mg intramuscular injection,four times a day.The experimental group was combined with modified Dachengqi decoction,1 dose per day,3 days as a course of treatment.After treatment,time of oral feeding,time of anal defecation,length of hospital stay,plasma gastrin(MOT),vasoactive intestinal peptide(VIP),somatostatin(SS),substance P(SP)and incidence of adverse reactions were compared between the two groups.Results After treatment,compared with before treatment,MOT,SS,SP increased and VIP decreased in both groups,and the experimental group was better than the control group(P<0.05).After treatment,compared with the control group,the experimental group had shorter oral feeding time,anal defecation time,hospitalization time,lower incidence of intestinal obstruction and adverse reactions(P<0.05).Conclusion Modified Dachengqi decoction can promote the recovery of intestinal function,shorten the recovery time of exhaust and defecation,effectively reduce postoperative complications and be suitable for clinical application.
