摘要
目的:比较联合激光与虹膜周切术治疗原发性闭角型青光眼(PACG)患者的临床效果。方法:回顾性分析2019年8月-2021年8月九江学院附属医院眼科95例PACG患者的临床资料,根据手术方式不同分为A组(虹膜周切术治疗,47例)与B组(联合激光治疗,48例)。对比两组患者术前、术后1周时前房相关参数[前方深度(ACD)、小梁虹膜夹角(TIA)、房角开放距离(AOD)],虹膜相关参数[虹膜膨隆度(IrisC)、虹膜厚度500(IT500)、虹膜厚度1 500(IT1 500)、虹膜根部附着点距离(IrisD)],并记录患者术后1周内的并发症发生情况。结果:术后1周,两组ACD、TIA、AOD均升高,且B组ACD、TIA、AOD均高于A组,差异均有统计学意义(P<0.05);术后1周,两组IrisC、IT500、IT1 500均降低,IrisD均升高,且B组IrisC、IT500、IT1 500均低于A组,IrisD高于A组,差异均有统计学意义(P<0.05);术后1周,A组并发症发生率高于B组,差异有统计学意义(P<0.05)。结论:较虹膜周切术,联合激光治疗PACG患者的效果更为显著,可改善前房相关参数及虹膜相关参数,降低术后并发症。
Objective:To compare the clinical effect of combined laser and peripheral iridotomy in the treatment of primary angle closure glaucoma (PACG).Method:The clinical data of 95 patients with PACG in Department of Ophthalmology,Affiliated Hospital of Jiujiang University from August 2019 to August 2021 were analyzed retrospectively.According to different surgical methods,they were divided into group A (iridectomy,47 cases) and group B (combined laser treatment,48 cases).The anterior chamber related parameters[anterior chamber depth (ACD),trabecular-iris angle (TIA),angle open distance (AOD)]and iris related parameters[iris curvature (IrisC),iris thickness 500 (IT500),iris thickness 1 500 (IT1 500),iris root attachment point distance (IrisD)]were compared of patients in both groups before operation and 1 week after operation,and the complications of patients within 1 week after operation were recorded.Result:One week after operation,the values of ACD,TIA and AOD in both groups were increased,and the values of ACD,TIA and AOD in group B were higher than those in group A,with statistical significant differences (P<0.05);one week after operation,the values of IrisC,IT500 and IT1 500 were decreased and the values of IrisD were increased in both groups.The values of IrisC,IT500 and IT1 500 in group B were lower than those in group A,and the value of IrisD was higher than that in group A,with statistical significant differences (P<0.05);one week after operation,the incidence of complications in group A was higher than that in the control group,with a statistical significant difference (P<0.05).Conclusion:Compared with peripheral iridotomy,the effect of combined laser treatment in patients with PACG is more significant,which can improve the relevant parameters of anterior chamber and iris related parameters,and reduce postoperative complications.
