超声生物显微镜在正常眼压窄角眼房角检查中的应用价值 被引量:2

The significance of application of ultrasound biomicroscopy in inspection of narrow intraocular angle with normal intraocular pressure

摘要 目的探究超声生物显微镜在正常眼压窄角眼房角检查中的应用价值,了解窄角眼房角的解剖形态特征。方法以2017年1月至2018年6月于内蒙古医科大学附属医院接受治疗的92例(96眼)正常眼压老年患者为回顾性研究对象,根据房角镜检查结果将患者分为两组,其中检查为窄房角的42例患者(46眼)为观察组,宽房角的50例患者(50眼)为对照组。分别使用A超和超声生物显微镜对两组患者进行检查。比较两组患者A超检查下前房深度、晶体厚度、眼轴长度以及相对晶体位置;比较两组患者超声生物显微镜检查下前房深度、晶体厚度、睫状体厚度、小梁虹膜角、房角开放距离、虹膜厚度以及虹膜晶体接触距离;比较两组患者点1%匹罗卡品滴眼液前后眼前段结构变化。结果A超检查发现,观察组患者前房深度(2. 50±0. 28) mm,晶体厚度(4. 96±0. 47) mm,眼轴长度(21. 23±0. 87) mm,相对晶体位置(2. 18±0. 13);对照组患者前房深度(3. 40±0. 31) mm,晶体厚度(4. 21±0. 45) mm,眼轴长度(23. 50±0. 79) mm,相对晶体位置(2. 35±0. 11),两组数据比较差异具有统计学意义(P <0. 05)。超声生物显微镜检查发现,观察组患者前房深度、睫状体厚度、小梁虹膜角、房角开放距离以及虹膜厚度均低于对照组患者,虹膜晶体接触距离高于对照组患者,两组数据比较差异具有统计学意义(P <0. 05)。点1%匹罗卡品滴眼液前后两组患者超声生物检查镜结果比较差异均具有统计学意义(P <0. 05),两组患者点1%匹罗卡品滴眼液前后前房深度差值、小梁虹膜角差值以及房角开放距离差值比较差异具有统计学意义(P <0. 05)。结论正常眼压窄角眼房角表现为前房浅、晶体厚、眼轴变短、晶状体相对位置靠前、小梁虹膜角变窄等特点,使用超声生物显微镜能够对眼前段结构进行很好的检查。 Objective To explore the significance of application of ultrasound biomicroscopy in inspection of narrow angle of the eye with normal intraocular pressure,and to understand the anatomical features of narrow angle of eyes.Methods The clinical data of 92 elderly patients(96 eyes)with normal intraocular pressure treated in the affiliated hospital of Inner Mongolia Medical University during January 2017 to June 2018 were reviewed.These patients were divided into two groups according to the results of cornea examination.Forty-two patients(46 eyes)with narrow corner were allocated in observation group,and 50 patients(50 eyes)with wide angle were allocated in control group.Patients in these two groups were examined by using A-mode ultrasound and ultrasound biomicroscopy.The depth of anterior chamber,crystal thickness,axial length and relative crystal position were compared between these two groups.The anterior chamber depth,crystal thickness,ciliary body thickness,trabecular iris angle,and room were compared under ultrasound biomicroscopy.The angular opening distance,the thickness of iris and the contact distance of iris crystal were compared.The changes of anterior segment of eye before and after application of 1%pilocarpine eye drops were compared between these two groups.Results A hypersonography revealed that the depth of anterior chamber in observation group was(2.50±0.28)mm,the crystal thickness was(4.96±0.47)mm,the axial length was(21.23±0.87)mm,the relative crystal position was(2.18±0.13),and depth of anterior chamber depth in control group was(3.40±0.31)mm.The crystal thickness was(4.21±0.45)mm,the axial length was(23.50±0.79)mm,and the relative crystal position was(2.35±0.11).The difference between these two groups was statistically significant(P<0.05).Ultrasound biomicroscopy revealed that the depth of anterior chamber,ciliary body thickness,trabecular iris angle,angle of open angle,and iris thickness were lower in observation group than those of control group.The iris lens contact distance was higher
