摘要
目的探讨口腔颌面部间隙感染(OMSI)炎性介质C-反应蛋白(CRP)、超敏C-反应蛋白(hs-CRP)、降钙素原(PCT)、纤维蛋白原(FIB)、D二聚体(DD)的表达及其联合检测诊断效能。方法收集2018年5月-2020年5月于河北省眼科医院就诊的OMSI患者95例为感染组,无相关症状的同期非感染性腮腺良性肿瘤患者90例为未感染组,评估患者感染程度并检测病原菌类型,检测血清CRP、hs-CRP、PCT、FIB、DD表达水平,进行对比分析、相关性研究及诊断效能评价。结果感染组患者检出病原菌95株,其中革兰阳性菌57株占比60.00%,以金黄色葡萄球菌为主;与未感染组相比,感染组CRP、hs-CRP、PCT、FIB、DD表达水平升高(P<0.05),病原菌类型不同,指标水平存在组间差异,革兰阳性菌组指标平均水平稍高;感染组患者hs-CRP、PCT表达水平与感染类型存在相关性(P<0.05);受试者工作特征曲线(ROC)曲线下面积(AUC)评价其临床诊断价值,从小到大为FIB、CRP、DD、PCT、hs-CRP、联合检测,联合检测的AUC最高为0.881。结论CRP、hs-CRP、PCT、FIB、DD表达水平与OMSI存在相关性,各指标联合诊断对OMSI诊断的灵敏度、特异度高,对OMSI靶向治疗及预后具有重要意义。
OBJECTIVE To explore the expression of inflammatory mediators C-reactive protein(CRP),high-sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT),fibrinogen(FIB)and D-dimer(DD)in the patients with oral and maxillofacial space infection(OMSI)and analyze the diagnostic efficiency of joint detection of the above indexes.METHODS A total of 95 patients with OMSI who were treated in Hebei Eye Hospital from May 2018 to May 2020 were assigned as the infection group,meanwhile,90 non-infectious parotid benign tumor patients without related symptoms were chosen as the no infection group.The severity of infection was evaluated,the species of pathogens were identified,the expression levels of serum CRP,hs-CRP,PCT,FIB and DD were detected and compared between the two groups of patients,and the diagnostic efficiencies of the indexes and the joint detection were observed.RESULTS Totally 95 strains of pathogens were isolated from the patients of the infection group,57(60.00%)of which were gram-positive bacteria,and Staphylococcus aureus was the predominant species.The expression levels of CRP,hs-CRP,PCT,FIB and DD of the infection group were significantly higher than those of the no infection group(P<0.05).There were differences in the species of pathogens and the levels of indexes,and the average levels of the indexes of the gram-positive bacteria group were relatively high;the expression levels of hs-CRP and PCT of the patients in the infection group were associated with the type of infection(P<0.05).Receiver operating characteristic(ROC)curve analysis showed that the diagnostic values of the indexes and the joint detection,from low to high,were as the followsing:FIB,CRP,DD,PCT,hs-CRP and the joint detection;the AUC of the joint detection was the highest(0.881).CONCLUSION There is association between the expression levels of CRP,hs-CRP,PCT,FIB,DD and the OMSI;the sensitivity and specificity of the joint detection of the indexes are high in diagnosis of OMSI,which has great significance in targeted treatment of OMSI and asses
