摘要
目的分析口腔鳞状细胞癌(OSCC)患者首次根治性手术治疗后的生存情况及其影响因素。方法选取OSCC患者75例,均行首次根治性手术治疗,统计5年生存率。采用自制《OSCC患者预后效果》调查问卷,收集患者性别、年龄、发病部位、病理分化程度、有无复发、有无淋巴结转移等信息,统计分析预后生存的影响因素。结果75例患者术后5年生存率56.00%(42/75)。性别、年龄、发病部位、病程和有无吸烟、饮酒、高血压、糖尿病,与OSCC患者术后5年生存无明显关联性(P>0.05);临床分期、病理分化程度、有无复发、有无淋巴结转移是OSCC患者术后5年生存的影响因素(P<0.05)。logistic回归分析显示,临床分期Ⅲ~Ⅳ期、病理分化程度低、复发、淋巴结转移为OSCC患者术后5年生存的危险因素(P<0.05)。结论OSCC患者首次根治性手术治疗后,5年生存率不理想,临床分期高、病理分化程度低、复发和淋巴结转移均为OSCC患者预后的危险因素,临床应据此加强疾病监测和防控干预,以延长患者生存期。
Objective To analyze the survival influencing factors of the first radical surgical treatment in patients with oral squamous cell carcinoma(OSCC).Methods 75 OSCC patients recieved first radical surgical treatment,statistical 5 year survival rate,using the homemade prognostic effect of OSCC patients with questionnaire,collect gender,age,site,pathological differentiation degree,recurrence,lymph node metastasis,etc.,information,for statistical analysis.Results 75 patients in this group,5-year survival rate is 56.00%(42/75);Gender,age,site of onset,disease course and absence of smoking,alcohol consumption,hypertension,diabetes,No significant association was observed with the 5-year postoperative survival in patients with OSCC(P>0.05),Clinical stage,degree of pathological differentiation,absence of recurrence,and lymph node metastasis are factors influencing 5-year postoperative survival in patients with OSCC(P<0.05);The logistic regression analysis showed that,Clinical stage stage~,low degree of pathological differentiation,recurrence,and lymph node metastasis are risk factors for 5-year postoperative survival in patients with OSCC(P<0.05).Conclusion After the first curative surgical treatment of OSCC patients,the 5-year survival rate is not ideal,high clinical stage,low degree of pathological differentiation,recurrence and lymph node metastasis are risk factors for OSCC patients,and disease monitoring and control intervention should be strengthened to prolong patient survival.
