摘要
目的:探讨社区综合治疗及管理在骨关节炎患者中的应用效果及对康复的影响。方法:选取2018年3月-2020年3月北京市昌平区东小口社区卫生服务中心收治的90例膝骨关节炎患者作为研究对象,将其分为两组,各45例。对照组实施常规对症治疗,观察组在对照组基础上实施综合治疗及管理。比较两组治疗后美国特种外科医院(HSS)评分、Berg平衡量表(BBS)评分、6 min步行距离、健康调查简表(SF-36)评分及不良事件发生率。结果:治疗后,观察组疼痛、功能、肌力、稳定性、屈曲畸形、活动度评分均高于对照组,差异有统计学意义(P<0.05);治疗前,两组BBS评分、6 min步行距离比较,差异无统计学意义(P>0.05);治疗后,观察组BBS评分高于对照组,6 min步行距离长于对照组,差异有统计学意义(P<0.05);观察组生理功能、生理职能、情感职能、精神健康、精力、社会功能、一般健康状况、躯体疼痛评分均高于对照组,差异有统计学意义(P<0.05);研究期间,两组均未出现与研究相关的不良事件。结论:社区综合治疗及管理能够改善膝骨关节炎患者的关节功能,提高患者平衡能力与行走能力,进而提升其生活质量,值得推广应用。
Objective:To explore the application effect of community comprehensive treatment and management in patients with osteoarthritis and its influence on rehabilitation.Methods:A total of 90 patients with knee osteoarthritis admitted to Dongxiaokou Community Health Service Center of Changping District in Beijing from March 2018 to March 2020 were selected as the study subjects and divided into two groups,with 45 cases in each group.The control group was given conventional symptomatic treatment,and the observation group was given comprehensive treatment and management on the basis of control group.The Hospital for Special Surgery(HSS)score,Berg Balance Scale(BBS)score,6-min walking distance,Health Survey Short Form(SF 36)score and the incidence of adverse events were compared between the two groups after treatment.Results:After treatment,the scores of pain,function,muscle strength,stability,flexion deformity,and range of motion in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in BBS score and 6-min walking distance between the two groups(P>0.05).After treatment,the BBS score of the observation group was higher than that of the control group,and the 6-min walking distance was longer than that of the control group,and the difference was statistically significant(P<0.05).The scores of physiological function,physiological function,emotional function,mental health,energy,social function,general health status,and physical pain scores of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).During the study period,there were no adverse events related to the study in either group.Conclusion:Community comprehensive treatment and management can improve the joint function of patients with knee osteoarthritis,improve their balance ability and walking ability,and accordingly improve their quality of life,which is worthy of popularizatio
