摘要
目的:探讨股骨近端防旋髓内钉(PFNA)内固定与动力髋螺钉(DHS)对高龄股骨转子间骨折(IFF)患者的治疗效果。方法:回顾性分析东莞市横沥医院2015年1月-2020年12月收治的97例高龄IFF患者的临床资料,随机分为两组。对照组23例行DHS治疗,观察组74例行PFNA治疗。比较两组术后负重时间与骨折愈合时间、髋关节功能及并发症。结果:观察组术后负重时间、骨折愈合时间均短于对照组,两组比较差异有统计学意义(P<0.05)。术后,观察组髋关节功能评分高于对照组,并发症发生率低于对照组,两组比较差异有统计学意义(P<0.05)。结论:高龄IFF患者采用PFNA内固定效果优于DHS,能够缩短术后恢复时间,降低并发症,改善患者髋关节功能。
Objective:To investigate the therapeutic effect of proximal femoral anti-rotation intramedullary nail(PFNA)internal fixation and dynamic hip screw(DHS)on elderly patients with intertrochanteric fracture(IFF).Methods:The clinical data of 97 elderly patients with IFF admitted to Dongguan Hengli Hospital from January 2015 to December 2020 were retrospectively analyzed.They were randomly into two groups according to the treatment plan.23 cases in the control group were treated with DHS,and 74 cases in the observation group were treated with PFNA.The postoperative weight-bearing time,fracture healing time,hip function and complications were compared between the two groups.Results:The postoperative weight-bearing time and fracture healing time of the observation group were shorter than those of the control group,and the difference between the two groups was statistically significant(P<0.05).After operation,the hip joint function score of the observation group was higher than that of the control group,and the incidence of complications was lower than that of the control group,and the difference between the two groups was statistically significant(P<0.05).Conclusion:PFNA internal fixation is better than DHS in elderly patients with IFF,which can shorten postoperative recovery time,reduce complications,and improve hip joint function.
