摘要
目的探讨远红外照射对ICU使用去甲肾上腺素(NA)患者发生医院获得性压疮(HAPU)的预防作用。方法收集2018年3月至2020年3月南京大学医学院附属鼓楼医院ICU内60例使用NA的患者,随机分为对照组(n=30)和观察组(n=30)。对照组治疗期间采取标准压疮预防护理措施,观察组在此基础上对患者骶尾部、足跟部进行远红外线照射,每次45 min,3次/d。比较两组患者HAPU发生率、骶尾部和足跟部经皮氧分压(TcPO_(2))变化和患者护理满意情况。结果观察组HAPU发生率明显低于对照组(6.67%vs 26.67%,P<0.05),两组住ICU时间差异无统计学意义(P>0.05);干预后两组骶尾部和足跟部TcPO_(2)随着时间延长呈降低趋势(P<0.05),观察组骶尾部和足跟部TcPO_(2)整体高于对照组(P<0.05);观察组患者护理满意情况优于对照组,差异有统计学意义(P<0.05)。结论远红外线照射能改善ICU使用NA患者的受压局部微循环血流障碍,降低HAPU发生率,提高临床护理效果。
Objective To explore the preventive effect of far infrared rays(FIR)irradiation on hospital-acquired pressure ulcers(HAPU)in patients treated with norepinephrine(NA)in ICU.Methods From March 2018 to March 2020,60 patients treated with NA in ICU of Nanjing Drum Tower Hospital were enrolled and randomly divided into control group(n=30)and observation group(n=30).During treatment,control group was given standard preventive care measures for pressure ulcers,while observation group was given FIR irradiation at sacrococcygeal and heel sites on basis of control group(45 min/once,3 times/d).The incidence of HAPU,changes of transcutaneous oxygen pressure(TcPO_(2)) at sacrococcygeal and heel sites,and nursing satisfaction were compared between the two groups.Results The incidence of HAPU in observation group was significantly lower than that in control group(6.67%vs 26.67%,P<0.05),while there were no significant difference in stay time of ICU between two groups(P>0.05).TcPO_(2) at sacrococcygeal and heel sites was decreased with time in both groups after intervention(P<0.05).TcPO_(2) at sacrococcygeal and heel sites in observation group was higher than that in control group(P<0.05).The nursing satisfaction of observation group was better than that of control group(P<0.05).Conclusion FIR irradiation can improve the local microcirculation blood flow disorders at compressed sites in ICU patients treated with NA,reduce incidence of HAPU and improve clinical nursing effect.
