摘要
目的探讨分阶段渐进性康复训练对慢性阻塞性肺疾病(简称慢阻肺)机械通气患者深静脉血栓(DVT)形成及肌力恢复的影响。方法选取天津市第四中心医院2020年5月至2022年5月接收的106例行机械通气治疗的慢阻肺急性发作期患者,根据随机数字表分为对照组(给予常规DVT预防管理)与试验组(给予常规DVT预防管理+分阶段渐进性康复训练),各53例,观察两组的凝血指标变化、DVT发生率及肌力恢复情况。结果分阶段渐进性康复训练干预后,试验组的各项凝血指标水平均显著优于对照组(P<0.05);试验组的DVT发生率为3.77%,显著低于对照组的16.98%(P<0.05);试验组的肌力评分显著高于对照组(P<0.05)。结论对慢阻肺机械通气患者在常规DVT预防管理同时联合分阶段渐进性康复训练干预,能够明显改善机体凝血指标,防止机体DVT发生,促进机体肌力恢复。
Objective To investigate the effect of phased progressive rehabilitation training on deep vein thrombosis(DVT)formation and muscle strength recovery in patients with chronic obstructive pulmonary disease(COPD)undergoing mechanical ventilation.Methods A total of 106 patients with acute exacerbation of COPD who received mechanical ventilation treatment in Tianjin Fourth Central Hospital from May 2020 to May 2022 were selected and divided into control group(conventional DVT preventive management)and experimental group(conventional DVT preventive management+phased progressive rehabilitation training)according to the random number table,53 cases in each group.The changes of coagulation index,the incidence of DVT and the recovery of muscle strength were observed in the two groups.Results After phased progressive rehabilitation training intervention,the levels of coagulation indexes in the experimental group were significantly better than those in the control group(P<0.05).The incidence of DVT in the experimental group was 3.77%,which was significantly lower than 16.98%in the control group(P<0.05).The muscle strength score of the experimental group was significantly higher than that of the control group(P<0.05).Conclusion Routine DVT prevention and management combined with phased progressive rehabilitation training intervention for mechanically ventilated patients with COPD can significantly improve the body's coagulation index,prevent the occurrence of DVT,and promote the recovery of muscle strength.
