基于量化评估下的早期四级康复训练在行机械通气重症患者护理中的应用效果 被引量:2

摘要 目的探讨基于量化评估下的早期四级康复训练在行机械通气重症患者中的应用效果。方法选取2020年6月至2021年2月于华中科技大学同济医学院附属同济医院重症监护室(ICU)住院治疗并行机械通气的138例重症患者作为研究对象,将其依据随机数字表法划为对照组和观察组,每组69例。对照组患者采用ICU重症常规护理干预,观察组患者在对照组护理方案的基础上采用以量化评估为基础的早期四级康复训练。比较2组患者的血气指标、运动和认知功能、机械通气时间、ICU治疗时间及并发症发生率。结果2组干预后的pH值、动脉血氧分压(PaO_(2))均较干预前升高,动脉血二氧化碳分压(PaCO_(2))均较干预前降低(P均<0.05);观察组的上述指标水平均优于对照组(P均<0.05)。2组在干预后的运动功能和认知功能评分均高于同组干预前(P均<0.05),且观察组均较对照组高(P均<0.05)。观察组的机械通气时间及ICU治疗时间均短于对照组(P均<0.05),并发症总发生率低于对照组(P<0.05)。结论行机械通气的重症患者接受以量化评估为基础的早期四级康复训练可以显著改善患者的血气指标,提高其运动、认知功能,加快其康复进程,且降低并发症发生率。 Objective To explore the application effect of early four-stage rehabilitation training based on quantitative evaluation on critically ill patients undergoing mechanical ventilation.Methods A total of 138 critically ill patients who received mechanical ventilation in the intensive care unit(ICU)of Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology from June2020 to February 2021 were selected as research objects.They were assigned into control and observation groups by the use of random number table,with 69 cases in each group.The patients in the control group were given routine intensive care intervention in ICU,and the patients in the observation group were given early four-stage rehabilitation training based on quantitative evaluation on the basis of routine intensive care.The blood gas analysis parameters,motor and cognitive function,mechanical ventilation duration,ICU length of stay,and complication rate were compared between the two groups.Results After intervention,the pH value and arterial partial pressure of oxygen(PaO_(2))in both groups were higher than those before intervention,and the arterial partial pressure of carbon dioxide(PaCO_(2))was lower than those before intervention(all P<0.05).The above parameters in the observation group were better than those in the control group(all P<0.05).After intervention,the scores of motor function and cognitive function in both groups were higher than those before intervention(all P<0.05),and the scores of the observation group were higher than those of the control group(both P<0.05).The mechanical ventilation duration and ICU length of stay in the observation group were shorter than those in the control group(both P<0.05),and the total complication rate was lower than that in the control group(P<0.05).Conclusion Early four-stage rehabilitation training based on quantitative evaluation for critically ill patients undergoing mechanical ventilation can significantly improve the blood gas analysis parameters,motor and cognitive funct
