摘要
目的 研究巨细胞病毒(CMV)感染对胆道闭锁(BA)患儿外周血Toll样受体4(TLR4)/p38促分裂原活化的蛋白激酶(p38 MAPK)信号通路和肝脏损伤的影响。方法 选取2018年1月-2020年12月浙江大学医学院附属儿童医院儿内科98例BA患儿为研究对象,根据有无合并CMV感染分为感染组和非感染组,采用聚合酶链反应(PCR)荧光定量分析检测外周血TLR4和p38 MAPK mRNA,采用蛋白免疫印迹法检测TLR4和p38 MAPK蛋白表达水平,同时检测外周血炎症因子和肝功能指标。结果 98例BA患儿中合并CMV感染37例占比37.76%。感染组外周血TLR4、p38 MAPK mRNA和蛋白表达水平高于非感染组,感染组血清白细胞介素(IL)-1β和肿瘤坏死因子α(TNF-α)水平高于非感染组,IL-10水平低于非感染组,差异均有统计学意义(P<0.05);感染组血清丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)和总胆红素(TBIL)水平高于非感染组(P<0.05),胆碱酯酶(ChE)和白蛋白(ALB)水平低于非感染组(P<0.05),两组血清γ-谷氨酰基转移酶(γ-GT)和总胆汁酸(TBA)水平比较,无统计学差异。结论 CMV感染可促进BA患儿TLR4/p38 MAPK信号通路激活,加重肝细胞炎症反应和损伤。
OBJECTIVE To observe the influence of cytomegalovirus(CMV) infection on peripheral blood Toll-like receptor 4(TLR4)/p38 mitogen-activated protein kinase(p38 MAPK) signaling pathway and liver injury of the children patients with biliary atresia(BA). METHODS A total of 98 children with BA who were treated in department of pediatric internal medicine of the Children’s Hospital, Zhejiang University School of Medicine from Jan 2018 to Dec 2020 were recruited as the study subjects and divided into the infection group and the non-infection group according to the status of CMV infection. The peripheral blood TLR4 and p38 MAPK mRNA were detected by polymerase chain reaction(PCR) fluorescent quantitative analysis, the expression levels of TLR4 and p38 MAPK proteins were detected by Western Blot, meanwhile, the peripheral blood inflammatory factors and liver function indexes were detected. RESULTS Among the 98 children with BA, 37 were complicated with CMV infection, accounting for 37.76%. The expression levels of peripheral blood TLR4 and p38 MAPK mRNA and proteins were significantly higher in the infection group than in the non-infection group, the levels of serum interleukin(IL)--1β and tumor necrosis factor-α(TNF-α) of the infection group were higher than those of the non-infection group, the IL-10 level of the infection group was significantly lower than that of the non-infection group(P<0.05). The levels of serum alanine aminotransferase(ALT), aspartate aminotransferase(AST) and total bilirubin(TBIL) of the infection group were significantly higher than those of the non-infection group(P<0.05), the levels of cholinesterase(ChE) and albumin(ALB) of the infection group were significantly lower than those of the non-infection group(P<0.05), there were no significant differences in the levels of serum γ-glutamyltransferase(γ-GT) and total bile acid(TBA) between the two groups. CONCLUSION The CMV infection may promote the activation of TLR4/p38 MAPK signaling pathway in the children with BA and aggravate inflammatio
