摘要
目的 探究急性心肌梗死(AMI)合并肺部感染患者血清MicroRNA-21(miRNA-21)表达情况。方法 选取2018年2月-2020年10月海南医学院第一附属医院收治的AMI合并肺部感染患者80例为感染组,单纯AMI患者76例为未感染组,同期健康体检者57名为对照组。比较各组血清miRNA-21、炎性因子表达水平;建立过表达miRNA-21人心肌细胞(HCM)细胞株,分析心肌细胞凋亡情况。结果 感染组C-反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-1β和miRNA-21较未感染组与对照组高(P<0.05);Ⅱ级心功能患者血清CRP、TNF-α、IL-6、IL-1β和miRNA-21水平较Ⅲ级与Ⅳ级低(P<0.05)。miRNA-21过表达组(minic组)经TNF-α处理后,其心肌细胞凋亡率低于shRNA对照组和HCM组(P<0.05)。感染组中,混合感染类型患者的血清炎性因子较真菌感染与细菌感染高(P<0.05),细菌感染和真菌感染的血清miRNA-21水平低于混合感染(P<0.05);肺部感染程度低危患者的血清炎性因子、miRNA-21水平较中危与高危低(P<0.05)。结论miRNA-21可抑制TNF-α介导的心肌细胞凋亡,在AMI合并肺部感染患者的血清中表达水平较高,并与患者心功能、感染类型、感染严重程度及炎性因子相关。
OBJECTIVE To explore the expression of serum microRNA-21(miRNA-21) in patients with acute myocardial infarction(AMI) and pulmonary infection. METHODS A total of 80 patients with AMI and pulmonary infection who were admitted to the First Affiliated Hospital of Hainan Medical College from Feb 2018 to Oct 2020 were selected as infection group, 76 patients only with AMI were selected as non-infection group, and 57 healthy individuals underwent physical examination during the same period were selected as the control group. The expression levels of serum miRNA-21 and inflammatory factors were compared among each groups. Human cardiomyocyte(HCM) cell lines with miRNA-21 overexpression were constructed, and the apoptosis of cardiomyocytes was analyzed. RESULTS The levels of C-reactive protein(CRP), tumor necrosis factor α(TNF-α), interleukin-6(IL-6), interleukin-1β(IL-1β) and miRNA-21 in the infection group were significantly higher than those in the non-infection and the control group(P<0.05). The levels of serum inflammatory factors including CRP, TNF-α, IL-6 and IL-1β, and the levels of miRNA-21 of patients with grade II cardiac function were significantly lower than those of patients with grade III and IV cardiac function(P<0.05). After the treatment with TNF-α, the apoptotic rate of cardiomyocytes in miRNA-21 overexpression group(minic group) was lower than that in the shRNA control group and HCM group(P<0.05). In the infection group, the levels of serum inflammatory factors of patients with mixed infection were significantly higher than those of fungal and bacterial infection(P<0.05), while serum miRNA-21 levels of patients with bacterial infection and fungal infection were significantly lower than that of patients with mixed infection(P<0.05). The levels of serum inflammatory factors and miRNA-21 in patients with low-risk to infection were significantly lower than that of patients with immediate-risk and high-risk to infection(P<0.05).
