清热利湿解毒法治疗湿热证银屑病有效性及安全性meta分析

Meta analysis of efficacy and safety of heat-clearing and diuresis-promoting and detoxifying in the treatment of psoriasis with damp-heat syndrome

摘要 目的用meta分析评价清热利湿解毒法治疗湿热证银屑病的有效性与安全性。方法检索中国知网、万方、重庆维普、中国生物医学文献、PubMed、Cochrane Library、Embase等数据库,收集清热利湿解毒法治疗湿热证银屑病的随机对照试验,检索时间范围自建库起至2020年12月。由两名研究者独立选择文献并收集数据,按照Cochrane5.1手册依次进行入选文献的偏倚风险及质量评估,用State/SE15.1软件进行meta分析。结果最终纳入13篇研究,涉及953例银屑病患者。meta分析结果显示,治疗组的总有效率(比值比=3.03,95%置信区间:2.04~4.48,P<0.0001)、银屑病面积和严重程度指数评分(标准化均数差=-0.68,95%置信区间:-0.97~-0.39,P<0.0001)、不良反应(比值比=0.32,95%置信区间:0.17~0.60,P<0.0001)均优于对照组。结论清热利湿解毒法治疗湿热证银屑病可显著降低银屑病面积和严重程度指数评分,改善皮损,疗效确切,安全性高。 Objective To evaluate the efficacy and safety of heat-clearing and diuresis-promoting and detoxifying in the treatment of psoriasis with damp-heat syndrome by meta analysis.Methods China National Knowledge Infrastructure,Wanfang,Chongqing VIP,China Biomedical Literature Database,PubMed,Cochrane Library,Embase and other databases were searched to collect randomized controlled trials of heat-clearing and diuresis-promoting and detoxifying in the treatment of psoriasis with damp-heat syndrome from the establishment of the database to December 2020.Two researchers assessed the risk of bias and quality of selected literature according to the Cochrane5.1 manual.State/SE15.1 software was used for meta analysis.Results Finally,13 studies involving 953 patients with psoriasis were included.The results of meta analysis showed that compared with the control group,the total effective rate[odds ratio(OR)=3.03,95%confidence interval(CI):2.04-4.48,P<0.0001],psoriasis area and severity index(PASI)score(standardized mean difference=-0.68,95%CI:-0.97--0.39,P<0.0001)and adverse reactions of the treatment group(OR=0.32,95%CI:0.17-0.60,P<0.0001)were better than those of the control group.Conclusion The method of efficacy and safety of heat-clearing and diuresis-promoting and detoxifying in the treatment of psoriasis with damp-heat syndrome can reduce the PASI score and improve the skin lesions,with definite curative effect and high safety.
