摘要
目的观察寻常型银屑病中医证型与血清炎症因子富亮氨酸α-2糖蛋白-1(LRG1)、转化生长因子-β1(TGF-β1)水平,探讨其与银屑病面积和严重程度指数(PASI)评分的相关性。方法收集90例寻常型银屑病患者,其中血热证、血瘀证、血燥证各30例,ELISA检测血清LRG1、TGF-β1水平,并与30例正常对照组比较,同时进行患者PASI评分。结果寻常型银屑病患者血热证、血瘀证、血燥证血清LRG1、TGF-β1水平明显高于正常对照组(P<0.05)。血清LRG1水平血热证、血瘀证高于血燥证(P<0.05);血热证与血瘀证比较无统计学意义(P>0.05)。血清TGF-β1水平由高到低依次为血热证、血瘀证、血燥证(P<0.05)。患者PASI评分从高到低依次为血热证、血瘀证、血燥证(P<0.05),血清LRG1、TGF-β1水平与PASI评分呈正相关(P<0.01)。结论寻常型银屑病血热证、血瘀证、血燥证血清LRG1、TGF-β1水平升高,与PASI评分呈正相关。血热证、血瘀证、血燥证间血清LRG1、TGF-β1存在差异,提示寻常型银屑病中医辨证分型存在血清炎症细胞因子表达依据,可为中医辨证分型提供参考指标。
Objective To investigate the TCM syndrome differentiation from psoriasis vulgaris and the serum LRG1 and TGF-β1 levels and their correlation with psoriasis area and severity index(PASI)score.Methods Totally90 patients with psoriasis vulgaris were collected,including 30 cases of blood heat syndrome,30 cases of blood stasis syndrome,and 30 cases of blood dryness syndrome.The levels of serum LRG1 and TGF-β1 were measured by ELISA,and compared with 30 cases in normal control group.At the same time,the PASI scores of the patients were evaluated.Results The serum levels of LRG1 and TGF-β1 in patients with psoriasis vulgaris with blood heat syndrome,blood stasis syndrome,and blood dryness syndrome were higher than those of the normal control group(P<0.05).The serum levels of LRG1 in blood heat syndrome and blood stasis syndrome were higher than those in blood dryness syndrome(P<0.05);there was no statistical significance between blood heat syndrome and blood stasis syndrome(P>0.05).The serum levels of TGF-β1 from high to low were blood heat syndrome,blood stasis syndrome,and blood dryness syndrome(P<0.05).The PASI score of the patients from high to low were blood heat syndrome,blood stasis syndrome,and blood dryness syndrome(P<0.05).Serum LRG1 and TGF-β1 levels were positively correlated with PASI score(P<0.01).Conclusion The levels of serum LRG1 and TGF-β1 in psoriasis vulgaris with blood heat syndrome,blood stasis syndrome and blood dryness syndrome increase,which are positively correlated with the PASI score.The levels of serum LRG1 and TGF-β1 between the groups of blood heat syndrome,blood stasis syndrome and blood dryness syndrome are different,suggesting that there is a basis for the expression of serum inflammatory cytokines in TCM syndrome differentiation of psoriasis vulgaris,which would provide a reference index for the TCM syndrome differentiation.
