摘要
目的探讨Th1/Th2/Th17相关细胞因子在非节段型白癫风患者表达水平及其与疾病发生发展的关系。方法收集2020年5月-2021年1月新疆维吾尔自治区人民医院皮肤科住院的非节段型白癫风患者56例,同期健康体检者40例,采用流式细胞仪检测两组受试者血清中ThI[白细胞介素(IL)-2、IL-12p70、干扰素(IFN)-γ肿瘤坏死因子(TNF)-α],Th2(IL-4、IL-10),Th17(IL-1β、IL-6、IL-17)细胞因子水平,并分析其在白癫风患者的不同分期、分型、皮损面积、疾病活动程度等临床亚组中的表达差异。结果除IL-12p70和TNF-α外,其余7种细胞因子在白癫风患者中的表达水平均高于对照组(P<0.05)。IL-6、IFN-γ、IL-17与白癫风疾病活动度(VIDA)呈正相关,差异有统计学意义。IL-1β与皮损面.积严重程度呈正相关(P<0.05)。IL-6在白癫风甲状腺功能异常组中表达水平高于甲状腺功能正常组(P=0.03)。结论非节段型白癫风患者Th1/Th2/Th17细胞因子失衡可能在疾病的发生发展中起重要作用,IL-6、IFN-γ及IL-17与疾病活动度有关,IL-1β与皮损面积大小有关,可作为疾病活动监测的重要指标。
ObjectiveTo investigate the expression level of Th1/Th2/Th17 related cytokines in patients with non-segmental vitligo,and its relationship with the occurrence and development of the disease.Methods56 patients with non-segmental vitiligo hospitalized in the dermatology department of the People's Hospital of Xinjang Uygur Autonomous Region from May 2020 to January 2021 and 40 healthy subjects in the same period were collected.Flow cytometry was used to detected the serum levels of Th1(IL-2,IL-12p70,IFN-γ,TNF-α),Th2(IL-4,IL-10),Th17(IL-1β,IL-6,IL-17)cytokine in the two groups of subjects.And its expression differences in dfferent clinical subgroups were analyzed,such astheirdffrent stages,type,skinlesion area,diseaseactiviy and comorbidities,in patients with vitiligo.ResultsExcept forIL-12p70 and TNF-α,theexpression levels ofthe other seven cytokines in patients with vitiligo were higher than control group(P<0.05).IL-6,IFN-γ,IL-17 are positively correlated with disease activity(VIDA),and the association is statistically significant.IL-1βhas a significant positive correlation with the severity of skin lesions(P<0.05).The expression level ofIL-6 in the abnormal thyroid function group was higher than normal thyroid function group(P-0.03).ConclusionTh1/Th2/Th17 cytokine imbalance may play an important role in the occurrence and development of non-segmental vitiligo.IL-6,IFNα-and IL-17 are related to disease activity,and IL1βis related to the skin lesion size,suggesting their roles for monitoring disease activity.
