摘要
目的对比椎后路单开门椎管扩大成形术中采用丝线缝合固定及微型钛板内固定治疗多节段脊髓型颈椎病的疗效及对术后并发症的影响。方法采用随机数字表法将赣南医学院第二附属医院2017年6月—2019年9月收治的68例多节段脊髓型颈椎病患者分为参照组和观测组,参照组(n=34)在椎后路单开门椎管扩大成形术中采用丝线缝合固定治疗,观测组(n=34)在椎后路单开门椎管扩大成形术中采用微型钛板内固定治疗,对比2组治疗优良率、VAS评分、JOA评分、恢复情况、术后并发症发生率。结果2组术前VAS评分、JOA评分、颈椎活动度、椎管矢状径差异无统计学意义(P>0.05),观测组术后治疗优良率较参照组更高,且VAS评分更低,JOA评分、颈椎活动度、椎管矢状径更高,术后康复锻炼时间更短,术后并发症总发生率更低,差异有统计学意义(P<0.05)。结论多节段脊髓型颈椎病患者在椎后路单开门椎管扩大成形术中采用微型钛板内固定治疗有助于改善颈椎活动能力,减轻疼痛,促进康复,降低术后并发症发生,值得推广。
Objective To analyze the curative effect and influence on postoperative complications of multi-stage cervical spondylotic myelopathy treated with silk thread suture fixation and mini titanium plate internal fixation in single-door laminoplasty with cone posterior approach.Methods A random number table was used to divide 68 patients with multi-segment cervical spondylotic myelopathy admitted to our hospital from June 2017 to September 2019 into a reference group and an observation group.The reference group(n=34)was treated with silk thread suture fixation in the single-door laminoplasty of posterior cone,while the observation group(n=34)was treated with mini titanium plate internal fixation in the single-door laminoplasty of posterior cone.The two groups were compared with the excellent and good rate of treatment and VAS.Score,JOA score,recovery status,postoperative complication rate.Results There was no statistically significant difference in VAS score,JOA score,cervical spine range of motion,and spinal canal sagittal diameter between the two groups(P>0.05).The observation group had a higher postoperative treatment excellent rate than the reference group,and the VAS score was lower.JOA The scores,cervical spine range of motion,and spinal canal sagittal diameter were higher,the postoperative rehabilitation exercise time was shorter,and the total incidence of postoperative complications was lower.The difference was statistically significant(P<0.05).Conclusion Internal fixation with mini titanium plate in single-door laminoplasty for patients with multilevel cervical spondylotic myelopathy is helpful to improve the mobility of cervical spine,relieve pain,promote rehabilitation and reduce postoperative complications,which is worthy of promotion.
