手术治疗脊髓型颈椎病对患者神经功能康复的效果

The Effect of Surgical Treatment of Cervical Spondylotic Myelopathy on the Rehabilitation of Patients' Nerve Function

摘要 目的研究脊髓型颈椎病的CT影像特征及手术治疗对神经功能康复的效果.方法选取该院2019年1月—2020年7月收治的30例脊髓型颈椎病患者为研究对象,在观察CT影像特征的基础上为其行手术治疗,观察手术治疗效果.结果术后1年随访到的28例患者中,远期疗效优良者共23例,治疗优良率为76.67%.年龄、病程、术前日本骨科协会评估治疗(Japanese Orthopaedic Association scores,JOA)评分、三节段/单节段是患者手术远期疗效的影响因素(P<0.05).手术治疗后,患者的运动、触觉、针刺感神经功能评分均高于手术前,差异有统计学意义(P<0.05).结论在了解患者影像学特征的基础上为其行手术治疗,有助于加速患者的神经功能康复,提高远期疗效,值得推广应用. Objective To study the CT imaging characteristics of cervical spondylotic myelopathy and the effect of surgical treatment on nerve function rehabilitation.Methods 30 patients with cervical spondylotic myelopathy admitted to the hospital from January 2019 to July 2020 were selected as the research objects.On the basis of observing the characteristics of CT images,surgical treatments were performed for them.The effects of surgical treatment were observed.Results Among the 28 patients who were followed up one year after the operation,23 patients had excellent long-term curative effects,and the excellent and good treatment rate was 76.67%.Age,course of disease,preoperative Japanese Orthopaedic Association(JOA)score,three-segment/single-segment are the factors influencing the long-term outcome of patients'surgery(P<0.05).After surgical treatment,the patient's neurological function scores such as movement,touch and acupuncture sensation were all higher than those before surgery,and the difference was statistically significant(P<0.05).Conclusion The surgical treatment on the basis of understanding the patient's imaging characteristics will help accelerate the patient's neurological recovery and improve the long-term efficacy.It is worthy of popularization and application.
