摘要
目的 分析有创机械通气治疗ICU重症心力衰竭患者的疗效。方法 选取我院于2017年6月-2018年7月收治的70例ICU重症心力衰竭患者的临床资料进行分析,根据治疗方法不同分为对照组(常规治疗)和观察组(在对照组的基础上加用有创机械通气治疗),分析2组患者治疗后的效果。结果 观察组患者总有效率为91.43%,对照组为71.43%,观察组患者总有效率高于对照组(P<0.05)。2组治疗前N末端B型利钠肽原(NT-proBNP)、心肌肌钙蛋白I(cTnl)、肌酸激酶同工酶(CK-MB)、乳酸脱氢酶(LDH)水平比较不具有统计学意义(P>0.05)。观察组治疗后NT-proBNP、cTnl、CK-MB、LDH水平低于对照组(P<0.05)。2组治疗前血气指标、心功能水平比较不具有统计学意义(P>0.05)。观察组治疗后,血气指标优于对照组,舒张末内径(LVEDD)、左心室收缩末内径(LVESD)水平低于对照组,射血分数(LVEF)水平高于对照组(P<0.05)。结论 有创机械通气治疗ICU重症心力衰竭患者疗效显著,可降低患者NT-proBNP、心肌指标水平,改善血气指标和心功能。
Objective To analyze the effect of invasive mechanical ventilation on ICU patients with severe heart failure.Methods The clinical data of 70 ICU patients with severe heart failure admitted to our hospital from June 2017 to July 2018 were selected for analysis.According to different treatment methods,the patients were divided into control group(conventional treatment)and observation group(invasive mechanical ventilation therapy was added on the basis of the control group)to analyze the effect of the two groups after treatment.Results The total effective rate of the observation group was 91.43%,and that of the control group was 71.43%.The total effective rate of the observation group was higher than that of the control group(P<0.05).There was no statistical significance(P>0.05)in comparison with LDH before treatment of N-terminal b-type natriuretic peptide progenitor(NT-proBNP)cardiac troponin I(cTnl)in the two groups(P<0.05).After treatment,the level of nt-probnp cTnl ck-mb LDH in the observation group was lower than that in the control group(P<0.05).Before treatment,blood gas index and cardiac function were not statistically significant(P>0.05).After treatment,blood gas index was superior to the control group,left ventricular systolic end diameter(LVEDD)was lower than the control group,and ejection fraction(LVEF)was higher than the control group(P<0.05).Conclusion Invasive mechanical ventilation is effective in the treatment of ICU patients with severe heart failure,which can reduce the myocardial index of nt-probnp and improve the blood and gas index and cardiac function.
