摘要
目的:研究聚焦超声消融猪皮肤组织的量效关系,探讨组织特性对浅表组织超声消融的影响。方法:选用离体猪皮肤组织和新鲜离体牛肝为实验材料,分别在聚焦超声消融辐照深度3 mm、4.5 mm条件下,控制自由场空间峰值声强为1 700~4 100 W/cm~2和辐照时间为20~120 ms进行聚焦超声消融实验,分析获得声能量、辐照声强、辐照时间、辐照深度与组织损伤安全及有效的量效关系;结合离体牛肝辐照实验,探讨不同组织特性对超声消融的影响。结果:辐照深度为3 mm时声能量1.3 J和2.3 J时声能量辐照产生的组织损伤面积分别为(0.37±0.03)mm~2和(0.96±0.12)mm~2,辐照深度为4.5 mm时声能量2.1 J和2.8 J时声能量辐照产生的组织损伤面积分别为(0.52±0.02)mm~2和(1.58±0.34)mm~2。随着声能量的增加猪皮肤组织安全有效的损伤面积逐渐增大,与辐照时间呈近似线性关系,与辐照声强呈非线性关系;随着辐照深度的增加,猪皮肤组织产生损伤的阈值增大(3 mm时为1.3 J,4.5 mm时为2.1 J),损伤面积减小(起始损伤3 mm时为0.96 mm~2,4.5 mm时为0.52 mm~2)。聚焦超声消融牛肝产生损伤的阈值和焦点偏移程度均小于猪皮肤组织,具有与皮肤组织不同的量效关系。结论:聚焦超声消融浅表组织具有安全、有效的量效关系,但仍然需要考虑组织特性的影响。
Objective:To study the dose-effect relationship of focused ultrasound ablation on porcine skin tissue so as to determine the effect of tissue characteristics on ultrasound ablation in superficial tissue.Methods:The in vitro porcine skin tissue and in vitro fresh bovine liver were selected as experimental material.The focused ultrasound ablation experiment was conducted under the radiation exposure depths with 3 mm and 4.5 mm in focused ultrasound ablation,and the sound intensity of spatial peak value of controlled free-field was 1700-4100 W/cm~2 and the radiation exposure time was 20-120 ms.In addition,the dose-effect relationships between the obtained sonic energy,the sound intensity of radiation exposure,the time of radiation exposure,the depth of exposure and the safety of tissue damage and effectiveness were further analyzed.And then,the radiation exposure experiment of in vitro bovine liver was combined to determine the effect of the characteristics of different tissue on ultrasound ablation.Results:The areas of damaged tissues,that caused by the radiation exposure of sound energy with 1.3J and 2.3J when the depth of radiation exposure was 3 mm,were(0.37±0.03) mm~2 and(0.96±0.12) mm~2,respectively.And the areas of damaged tissues,that caused by the radiation exposure of sound energy with 2.1J and 2.8J when the depth was 4.5mm,were(0.52±0.02)mm~2 and(1.58±0.34) mm~2,respectively.The damaged area of safe and effective porcine skin tissue gradually enlarged with the increasing of sound energy,and the enlargement appeared approximate linear relationship with the time of radiation exposure,which appeared non-linear relationship with the sound intensity of radiation exposure.The threshold value that caused damage of porcine skin tissue enlarged with the increasing of the depth of radiation exposure(it was 1.3 J at 3mm,and it was 2.1J at 4.5mm),and the damaged area reduced with that(it was 0.96mm~2at original damage with 3mm,and it was 0.52mm~2 at 4.5mm).The excursion degrees both threshold value and focus poin
