摘要
目的分析泛耐药鲍曼不动杆菌重症肺炎患者行不同剂量替加环素治疗的临床效果。方法74例泛耐药鲍曼不动杆菌重症肺炎患者,根据随机数字编号末尾数的奇偶性分为对照组(奇数)和观察组(偶数),每组37例。对照组患者给予替加环素常规剂量治疗,观察组患者给予替加环素增加剂量治疗。比较两组患者临床治疗效果及治疗前后炎症指标(白细胞计数、降钙素原及C反应蛋白)。结果治疗后,两组患者白细胞计数、降钙素原及C反应蛋白均较本组治疗前明显下降,且观察组患者白细胞计数(4.35±0.63)×10^(9)/L、降钙素原(0.24±0.08)μg/L、C反应蛋白(3.49±1.01)mg/L均低于对照组的(8.23±0.92)×10^(9)/L、(0.94±0.07)μg/L、(11.12±2.01)mg/L,差异具有统计学意义(P<0.05)。观察组患者治疗总有效率为94.59%,高于对照组的78.38%,差异具有统计学意义(P<0.05)。结论采用替加环素对泛耐药鲍曼不动杆菌重症肺炎患者进行治疗时,酌情予以剂量增加,能切实帮助患者减轻炎症反应,提升疾病疗效,值得临床进一步研究推广。
Objective To analyze the clinical effect of different doses of tigecycline in the treatment of severe pneumonia caused by pan-drug resistant Acinetobacter baumannii.Methods A total of 74 patients with severe pneumonia caused by pan-drug resistant Acinetobacter baumannii were divided into control group(odd number)and observation group(even number)according to the parity of the numbers at the end of the number,with 37 cases in each group.Patients in the control group were treated with conventional dose of tigecycline,and patients in the observation group were treated with increased dose of tigecycline.The clinical effect and inflammatory indexes(white blood cell count,procalcitonin and C-reactive protein)before and after treatment were compared between the two groups.Results After treatment,the white blood cell count,procalcitonin and C-reactive protein in the two groups were significantly decreased compared with those before treatment in this group;the white blood cell count in the observation group was(4.35±0.63)×10^(9)/L,and the procalcitonin was(0.24±0.08)μg/L,C-reactive protein was(3.49±1.01)mg/L,which were lower than(8.23±0.92)×10^(9)/L,(0.94±0.07)μg/L,(11.12±2.01)mg/L in the control group.All the differences were statistically significant(P<0.05).The total effective rate in the observation group was 94.59%,which was higher than 78.38%in the control group,and the difference was statistically significant(P<0.05).Conclusion when tigecycline is used in the treatment of severe pneumonia caused by pan-drug resistant Acinetobacter baumannii,increasing the dose as appropriate can effectively help patients reduce the inflammatory reaction and improve the efficacy of the disease,which is worthy of further clinical research and promotion.
