超声龈下洁治联合米诺环素治疗慢性牙周炎的临床效果及对牙周指数的影响 被引量:2

Clinical Effect of Ultrasonic Subgingival Scaling Combined with Minocycline in the Treatment of Chronic Periodontitis and Its Influence on Periodontal Index

摘要 目的:探讨超声龈下洁治联合米诺环素治疗慢性牙周炎(CP)的临床效果及对牙周指数的影响。方法:选取2019年7月-2021年7月江西省中西医结合医院收治的92例CP患者,随机分为对照组(46例)和观察组(46例)。两组均行超声龈下洁治,对照组予以甲硝唑治疗,观察组予以米诺环素治疗,持续治疗4周,之后随访3个月。比较两组临床疗效、牙周指数、炎症因子水平、不良反应及复发率。结果:相比对照组,治疗4周后观察组总有效率较高,随访3个月复发率较低,差异均有统计学意义(P<0.05);观察组治疗4周后龈沟出血指数(BI)、探诊深度(PD)、菌斑指数(PLI)及牙龈指数(GI)分别为(1.05±0.25)分、(2.05±0.28)mm、(1.12±0.28)分、(1.19±0.22)分,均低于对照组的(1.35±0.29)分、(2.64±0.37)mm、(1.41±0.32)分、(1.42±0.27)分,差异均有统计学意义(P<0.05);观察组治疗4周后C反应蛋白(CRP)[(4.63±1.02)ng/mL]、白介素-6(IL-6)[(72.13±6.38)μg/L]、肿瘤坏死因子-α(TNF-α)[(81.59±7.12)μg/L]均低于对照组[(6.61±1.28)ng/mL、(80.24±6.59)μg/L、(90.37±7.58)μg/L],差异均有统计学意义(P<0.05);对照组出现3例不良反应,观察组出现2例不良反应,组间不良反应发生率差异无统计学意义(P>0.05)。结论:米诺环素联合超声龈下洁治可提高CP治疗效果,减轻牙周炎症反应,改善牙周组织状况,减少疾病复发,安全可靠。 Objective:To investigate the clinical effect of ultrasonic subgingival scaling combined with Minocycline in the treatment of chronic periodontitis(CP)and its influence on periodontal index.Method:A total of 92 patients with CP admitted to Jiangxi Hospital of Integrated Traditional Chinese and Western Medicine from July 2019 to July 2021 were selected and randomly divided into control group(46 cases)and observation group(46 cases).Ultrasonic subgingival scaling was performed in both groups,the control group was treated with Metronidazole,and the observation group was treated with minocycline.The treatment lasted for 4 weeks,followed by a 3-month follow-up.Clinical efficacy,periodontal index,levels of inflammatory factors,adverse reactions and recurrence rates were compared between the two groups.Result:Compared with the control group,the total effective rate of the observation group was higher after 4 weeks of treatment,and the recurrence rate was lower after 3 months of follow-up,with statistical significance(P<0.05).After 4 weeks of treatment,BI,PD,PLI and GI were(1.05±0.25)points,(2.05±0.28)mm,(1.12±0.28)points,(1.19±0.22)points,lower than(1.35±0.29)points,(2.64±0.37)mm,(1.41±0.32)points,(1.42±0.27)points of the control group,the differences were statistically significant(P<0.05);the levels of C reactive protein(CRP)[(4.63±1.02)ng/mL],interleukin-6(IL-6)[(72.13±6.38)μg/L]and tumor necrosis factor-α(TNF-α)[(81.59±7.12)μg/L]in the observation group were lower than(6.61±1.28)ng/mL,(80.24±6.59)μg/L,(90.37±7.58)μg/L in the control group,the differences were statistically significant(P<0.05).The adverse reactions were 3 cases in the control group and 2 cases in the observation group,there was no significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion:Minocycline combined with ultrasonic subgingival scaling can improve the therapeutic effect of CP,reduce periodontal inflammation,improve periodontal tissue status,and reduce disease recurrence,which is safe
