专业化健康教育联合心理干预对鼻内镜下慢性鼻窦炎患者围手术期的影响研究 被引量:26

Effect of professional health education combined with psychological intervention on the perioperative period of nasal endoscopy surgery in patients with chronic rhinosinusitis

摘要 目的探讨专业化健康教育联合心理干预对鼻内镜下慢性鼻窦炎患者围手术期的影响。方法收集安康市人民医院2018年6月至2021年5月行鼻内镜下慢性鼻窦炎围手术期患者124例作为研究对象,随机分为对照组62例和观察组62例。两组患者均行常规慢性鼻窦炎术后护理,观察组另行专业化健康教育联合心理干预;对比两组患者术后症状积分、自我管理能力、心理状态及疼痛程度。结果术后3个月,观察组鼻腔通气(2.52±0.76)分、鼻涕(1.36±0.21)分、鼻腔干燥(2.09±0.33)分、头痛(1.54±0.24)分,均明显低于对照组各项得分[(3.18±0.65)分、(1.51±0.25)分、(2.52±0.41)分、(1.73±0.33)分](P均<0.05)。干预1周后,观察组健康行为(8.83±1.37)分、情绪管理(9.03±1.02)分、依从性(9.24±1.15)分,均明显高于对照组各项得分[(7.21±1.85)分、(7.25±1.86)分、(8.18±1.57)分](P均<0.05)。干预1周后,观察组焦虑自评量表(self-rating anxiety scale,SAS)评分为(36.24±2.93)分、抑郁自评量表(self-ratingdepression scale,SDS)评分为(37.18±3.04)分,均明显低于对照组各项得分[(45.16±3.27)分、(47.27±3.26)分](P均<0.05)。观察组术后第1天、第3天疼痛评分分别为(5.84±1.62)、(2.63±0.87)分,均明显低于对照组各项得分[(6.53±1.42)分、(4.14±1.28)分](P均<0.05)。结论专业化健康教育联合心理干预明显改善鼻内镜下慢性鼻窦炎患者术后症状、自我管理能力、心理状态,并降低患者术后疼痛程度。 Objective To investigate the effect of specialized health education combined with psychological intervention on the perioperative period of nasal endoscopy surgery in patients with chronic rhinosinusitis.Methods A total of 124 patients with chronic sinusitis under nasal endoscopy in Ankang People's Hospital from June 2018 to May 2021 were collected as the research objects,and randomly divided into the control group of 62 cases and the observation group of 62 cases.Both groups were given routine postoperative care for chronic sinusitis,and the observation group was additionally given professional health education combined with psychological intervention;The postoperative symptom scores,self-management ability,psychological state and pain degree were compared between the two groups.Results Three months after surgery,the postoperative scores in nasal ventilation,nasal discharge,nasal dryness,and headache in the observation group were significantly lower(P<0.05),compared with the control group symptom scores[(2.52±0.76 vs 3.18±0.65),(1.36±0.21 vs 1.51±0.25),(2.09±0.33 vs 2.52±0.41),(1.54±0.24 vs 1.73±0.33),respectively].One week after intervention,the scores of health behavior,emotion management and compliance in the observation group were signifcantly higher than those in the control group[(8.83±1.37 vs 7.21±1.85),(9.03±1.02 vs 7.25±1.86),(9.24±1.15 vs 8.18±1.57),respectively](all P<0.05).After 1 week of intervention,the SAS and SDS scores of the observation group were remarkably lower than those of the control group(36.24±2.93 vs 45.16±327 points,37.18±3.04 vs 47.27±3.26,respectively)(all P<0.05).On the 1st and 3rd day after operation,the pain scores of the observation group were suirprisingly lower than those of the control group(5.84±1.62 vs 6.53±1.42,2.63±0.87 vs 4.14±1.28,respectively)(all P<0.05).Conclusions Professional health education combined with psychological intervention can significantly improve postoperative symptoms,self management ability and psychological state of patients wit
