摘要
目的:探讨伴冲动暴力行为的精神分裂症(SCH)患者认知功能损害与神经影像学改变的关系。方法:将84例SCH患者分为冲动暴力组40例和非冲动暴力组44例;采用数字划销测验(CT)、修订韦氏成人记忆量表(WMS-RC)和威斯康星卡片分类测验(WCST)检测其认知功能;采用功能性磁共振成像(fMRI)采集入组者低频振幅(ALFF)及分数低频振幅(FALFF),分析两组间差异的脑区分布;对冲动暴力组各量表评分与ALFF和FALFF值的关系进行Pearson相关分析。结果:冲动暴力组CT、WMS-RC及WCST评分明显低于非冲动暴力组(P均<0.01);冲动暴力组右侧顶区ALFF值显著低于非冲动暴力组,小脑下部FALFF值显著高于非冲动暴力组(P均<0.01);ALFF值与CT、WMS-RC和WCST评分呈正相关,FALFF值与CT、WMS-RC和WCST评分呈负相关(P<0.05或P<0.01)。结论:伴有冲动行为的SCH患者认知功能损害更严重,并与其右侧顶区及小脑下部的脑功能改变有关。
Objective:To explore the relationship between cognitive impairment and neuroimaging changes in schizophrenic(SCH)patients with impulsive violence.Method:84 patients with SCH were divided into violence group(n=40)and non-violence group(n=44).The cognitive function was tested by cancellation test(CT),Wechsler Memory Scale-Revised in China(WMS-RC)and Wisconsin Card Sorting Test(WCST).The amplitude of low frequency fluctuations(ALFF)and fractional amplitude of low frequency fluctuations(FALFF)were collected by functional magnetic resonance imaging(fMRI),and the distribution of brain regions with differences between the two groups was analyzed.The relationship between CT,WMS-RC,WCST scores and ALFF,FALFF in violence group was analyzed by Pearson correlation analysis.Results:The scores of CT,WMS-RC and WCST in violence group were significantly lower than those in non-violence group(all P<0.01).ALFF value of right parietal region in violence group was significantly lower while FALFF value of inferior cerebellum was significantly greater than those in non-violence group(P<0.05).ALFF was positively correlated with CT,WMS-RC and WCST scores,while FALFF was negatively correlated with them(P<0.05 or P<0.01).Conclusion:The cognitive impairment is more severe in SCH patients with impulsive violence;which is related to the changes of brain function in the right parietal region and lower cerebellum.
