摘要
目的检索并获取老年股骨转子间骨折术后快速康复运动干预的相关证据,并对最佳证据进行总结。方法运用循证的护理方法,从加速康复外科理念出发,针对老年股骨转子间骨折术后运动干预方案,检索相关文献,采用澳大利亚JBI循证卫生保健中心的文献评价标准和证据分级系统对文献进行质量评价和证据级别评定。检索时限自建库截止到2020年12月30号。结果最终纳入12篇文献,包括3篇指南、1篇专家共识、3篇系统评价、2篇临床随机对照试验、2篇回顾性病例对照试验及1篇临床决策。从多学科团队协作、运动评估、运动时机与方式、运动依从性、运动原则5个维度归纳了19条最佳证据。结论建议临床医务人员在加速康复外科的理念指导下,遵循个体化、循序渐进和全面锻炼的原则尽早开展运动干预,以促进患者快速康复,提升临床护理质量。
Objective To search and obtain the evidences on postoperative exercise therapy of geriatric femoral intertrochanteric fractures, and to summarize the best available evidence. Methods The method of evidence-based nursing was used to search literatures of postoperative exercise therapy programs for geriatric femoral intertrochanteric fractures on the basis of enhance recovery after surgery(ERAS).Literature evaluation criteria and evidence classification system of Australian JBI Evidence-Based Health Care Center were used to conduct quality evaluation and evidence rating for these literatures. The retrieval time limit was due to be on December 30, 2020. Results A total of 12 articles were selected, including 3 guidelines, 1 expert consensus,3 systematic evaluation, 2 randomized controlled trails,2 retrospective case-control studies, and1 clinical decision. Totally 19 best evidences were summarized from 5 dimensions of multidisciplinary cooperation, exercise evaluation, exercise timing and ways, exercise compliance, and exercise principle. Conclusion It is suggested that the clinical medical staff will implement exercise therapy as early as possible in the principle of accelerating rehabilitation surgery, step by step as well as comprehensive exercise followed the guidance of ERAC, so as to promote patients fast-track rehabilitation and improve the quality of clinical nursing.
