接种与未接种新型冠状病毒疫苗Omicron变异株感染患者临床表现、肝功能、抗体水平对比观察 被引量:3

摘要 目的 对比接种与未接种新型冠状病毒疫苗Omicron变异株感染患者的临床表现、肝功能、抗体水平。方法 选择天津市Omicron变异株感染的新型冠状病毒肺炎(COVID-19)恢复期患者430例,其中接种科兴疫苗150例(科兴组),接种北京生物疫苗185例(北京生物组),接种康希诺疫苗41例(康希诺组),接种安徽智飞、长春生物、兰州生物、山东生物、其他腺病毒载体疫苗以及混合接种16例(其他组),未接种疫苗38例(未接种组)。回顾性分析其临床资料,比较各组临床表现、肝功能相关指标[天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、总胆红素(TB)、白蛋白(ALB)、总蛋白(TP)、乳酸脱氢酶(LDH)]以及抗体(IgG、IgM)水平。结果 各组性别构成比比较P均>0.05;北京生物组年龄明显低于其他组,北京生物组、康希诺组末次接种疫苗至入院时间<3个月的比例明显高于科兴组及其他组(P均<0.01)。年龄<16岁的患儿共110例,科兴组、北京生物组、康希诺组、其他组、未接种组分别为7、88、0、1、14例,无症状感染6例、普通型13例、轻型91例、重型0例。北京生物组与未接种组ALT异常率比较无统计学差异(P>0.05),科兴组、康希诺组ALT异常率均高于未接种组与北京生物组(P均<0.05)。未接种组AST异常率均高于其他各组(P均<0.05)。各组AST、TP、TB比较差异均无统计学意义(P均>0.05)。科兴组及康希诺组ALT水平均高于北京生物组及未接种组,未接种组ALB水平低于其他各组,北京生物组LDH水平高于科兴组及康希诺组(P均<0.05)。未接种组IgG、IgM水平均低于科兴组、北京生物组、康希诺组(P均<0.05)。结论 接种北京生物生产新型冠状病毒疫苗的Omicron变异株感染患者的年龄较小、轻症比例较高、在一定程度上可保护患者的肝功能,接种不同新型冠状病毒疫苗后的患者感染新型冠状病毒后均可产生更高水平的IgG、IgM抗体。 Objective To compare the clinical manifestations,liver function,and antibody levels between Omicron variant infection patients vaccinated and not vaccinated with COVID-19 vaccine.Methods Totally 430 convalescent COVID-19 patients infected with Omicron variant in Tianjin were selected,including 150 patients vaccinated with C.orona Vaccine(Sinovac group),185 patients vaccinated with BBIBP-CorV(Beijing biological group),41 patients vaccinated with Ad5-nCoV vaccine(C.anSino group),16 patients vaccinated with Anhui Zhifei,Changchun Bio,Lanzhou Bio,Shandong Bio,other adenovirus vector vaccines or mixed vaccination(other group),and 38 unvaccinated patients(unvaccinated group).The clinical manifestations,liver function indexes [alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TB),albumin(ALB),total protein(TP),lactate dehydrogenase(LDH)],and antibody levels(IgG,IgM) were compared retrospectively.Results There was no statistical difference in the sex composition ratio among groups(P>0.05).The age of the Beijing biological group was significantly lower than that of other groups,and the proportion of time less than 3 months from the last vaccination to admission in the Beijing biological group and CanSino group was significantly higher than that in the Sinovac group and other groups(all P0.05),but the abnormal rates of ALT were higher in the Sinovac group and CanSino group than in the unvaccinated group and Beijing biological group(all P0.05).The levels of ALT were higher in the Sinovac group and CanSino group than in Beijing biological Group and unvaccinated group,the level of ALB in the unvaccinated group was lower than that in the other groups,and the level of LDH in the Beijing biological group was higher than those in the Sinovac group and CanSino group(both P<0.05).The IgG and IgM antibody levels of the unvaccinated group were significantly lower than those of the Sinovac group,Beijing biological group and CanSino group(all P<0.05).Conclusions Omicron variant infection patients vaccinated
