摘要
目的:很多神经精神疾病与婴幼儿的脑组织发育异常有关。本研究旨在分析磁共振波谱成像(magnetic resonance spectroscopy,MRS)联合弥散张量成像(diffusion tensor imaging,DTI)参数在2个月至2岁婴幼儿脑发育过程中的变化规律,探讨MRS联合DTI评价2个月至2岁正常婴幼儿脑发育的应用价值。方法:选取2020年9月至2021年5月在山西省儿童医院出生后接受全脑MRS和DTI检查的婴幼儿116例,根据年龄分为A组(2~6个月,7例)、B组(7~12个月,28例)、C组(13~18个月,41例)、D组(19~24个月,40例)。对MRS数据及DTI数据进行预处理后再行统计学分析,比较各组各脑区DTI参数和MRS代谢产物比值。结果:4组额叶、颞叶、枕叶、内囊后肢、内囊前肢、胼胝体膝部、胼胝体压部及视放射DTI参数比较差异有统计学意义(P<0.05或P<0.01)。从A组至D组各向异性分数(fractional anisotropy,FA)值呈上升趋势,表观扩散系数(apparent diffusion coefficient,ADC)、轴向扩散(axial diffusivity,AD)值和径向弥散(radial diffusivity,RD)值呈下降趋势,且参数的变化幅度随着年龄增长有减缓趋势。在左或右豆状核中,胆碱(choline,Cho)/肌酸(creatine,Cr)比值从A组到D组呈下降趋势,且D组显著低于A、B组(均P<0.01);Cho/N-乙酰天冬氨酸(N-acetyl aspartate,NAA)比值从A组到D组呈下降趋势,且D组显著低于A、B、C组(左豆状核,P<0.05或P<0.01)或A、B组(右豆状核,均P<0.01);谷氨酸类化合物(glutamine/glutamate,Glx)/Cr比值从A组到D组呈下降趋势,且D组显著低于A、B、C组(P<0.05或P<0.01);肌醇(myo-inositol,mI)/Cr比值从A组到D组呈上升趋势,且D组显著高于A、B、C组(P<0.05或P<0.01);NAA/Cr比值从A组到D组呈上升趋势,且B、C、D组显著高于A组(P<0.05或P<0.01)。A组至D组不同脑区的mI/Cr和NAA/Cr比值呈上升趋势,Cho/Cr、Cho/NAA和Glx/Cr呈下降趋势,且每个参数的变化幅度随着年龄增长有减缓趋势。结论:MRS和DTI检查可评价2个月~2岁婴幼�
Objective:Many neuropsychiatric diseases are related to the abnormal development of brain tissue in infants.This study aims to analyze the changes in the parameters of magnetic resonance spectroscopy(MRS)and diffusion tensor imaging(DTI)in brain development of infants aged from 2 months to 2 years old,and to explore the value of MRS combined with DTI in evaluating brain development of infants aged from 2 months to 2years old.Methods:A total of 116 normal infants,who received whole brain MRS and DTI examinations after delivery in Children Hospital of Shanxi Province from September 2020to May 2021,were selected and were divided into a group A(n=7,at the age of 2-6months),a group B(n=28,at the age of 7-12 months),a group C(n=41,at the age of 13-18 months),and a group D(n=40,at the age of 19-24 months).After collecting the MRS and DTI data,statistical analysis was performed to compare DTI parameters and MRS metabolic products ratio.Results:There were significant differences in the DTI parameters of frontal lobe,temporal lobe,occipital lobe,hind limb of internal capsule,fore limb of internal capsule,knee of corpus callosum,splenium of corpus callosum,and optic radiation among the 4 groups(P<0.05 or P<0.01).The values of fractional anisotropy(FA)showed an upward trend from the group A to the group D,while the values of apparent diffusion coefficient(ADC),axial diffusivity(AD),and radial diffusivity(RD)showed a downward trend,and the changes of parameters tended to slow down with age.In the left or right lentiform nucleus,the ratio of choline(Cho)/creatine(Cr)was decreased from the group A to the group D,and the group D was significantly lower than the group A and B(all P<0.01).The ratio of Cho/N-acetyl aspartate(NAA)was decreased from the group A to the group D,and the group D was significantly lower than the group A,B,and C(left lentiform nucleus,P<0.05 or P<0.01)or the group A,B(right lentiform nucleus,both P<0.01).The ratio of glutamine/glutamate(Glx)/Cr was decreased from the group A to the group D,and the group D
