摘要
目的分析颅内多发动脉瘤患者(MIA)中破裂的颅内动脉瘤(IA)的临床特征。方法回顾分析了2017—2021年安阳市人民医院神经外科312例开颅夹闭的破裂IA患者,依据IA的个数分为单发组和多发组,其中单发组241例,多发组71例;在多发组中,破裂动脉瘤为71个,未破裂动脉瘤106个,统计分析单发组与多发组组间临床特征的差异,以及多发组内破裂动脉瘤和未破裂动脉瘤的临床特征差异。结果多发组女性患者发病率高于单发组(P<0.05);单发组60岁以下患者高于多发组,多发组80岁以上患者高于单发组,差异有统计学意义(P<0.05)。在MIA中,破裂组动脉瘤最大直径<5 mm的比率小于未破裂组,最大直径5 mm以上的比率大于未破裂组(P<0.01);关于动脉瘤的形态,破裂组不规则率大于未破裂组(P<0.01);破裂组纵横比均值大于未破裂组(P<0.01);破裂组IA最大直径的均值大于未破裂组(P<0.01)。结论在MIA中可以使用纵横比预测和判定动脉瘤的破裂,形状不规则和IA最大直径在破裂动脉瘤的判断上也具有指导意义。
Objective To explored the clinical features of ruptured intracranial aneurysm(IA)in patients with multiple intracranial aneurysms(MIA).Methods A total of 312 cases of ruptured IA patients with craniotomy clipping in the neurosurgery department of Anyang People’s Hospital from 2017 to 2021 were retrospectively analyzed,including 241 cases of single IA and 71 cases of MIA.In the multiple aneurysm group,there were 71 ruptured aneurysms and 106 unruptured aneurysms.The differences in clinical features between the single and multiple groups,as well as between ruptured and unruptured aneurysms in multiple groups were statistically analyzed.Results Single IA was more common in patients under 60 years old and MIA was more common in patients over 80 years old and female patients(P<0.05).In MIA,most small aneurysms(<5 mm)were unruptured,while large aneurysms(<5 mm)were ruptured easily(P<0.01).As for the morphology of aneurysm,the irregularity rate of ruptured group was higher than that of unruptured group(P<0.01).The mean aspect ratio of the ruptured group was larger than that of the unruptured group(P<0.01).The mean maximum diameter of IA in the ruptured group was larger than that in the unruptured group(P<0.01).Conclusion It was feasible to use aspect ratio to determine ruptured aneurysms in MIA.Irregular shape and maximum diameter of IA were useful in the determination of ruptured aneurysms.
