摘要
目的:探讨康柏西普联合Ex-PRESS青光眼引流器对新生血管性青光眼(NVG)患者的影响。方法:选取2017年1月-2021年12月古田县医院及福建省立医院(进修期间)收治的110例120眼的NVG患者,根据随机投硬币法分为观察组和对照组,对照组55例(58眼),观察组55例(62眼)。两组在术前均行康柏西普玻璃体腔注射,观察组和对照组分别选择Ex-PRESS青光眼引流器植入治疗和Ahmed引流阀植入治疗。比较两组患者视力恢复情况、治疗前后眼压(IOP)变化、治疗有效率、房水和血清血管内皮生长因子(VEGF)、C反应蛋白(CRP)、血清白细胞介素-6(IL-6)水平。结果:(1)观察组治疗后3个月视力恢复率高于对照组,但组间差异无统计学意义(P>0.05)。(2)两组治疗后IOP比较,差异无统计学意义(P>0.05)。(3)治疗后,观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。(4)治疗后1个月,两组房水和血清VEGF、CRP、IL-6水平均低于治疗前(P<0.05),且观察组低于对照组,差异有统计学意义(P<0.05)。结论:康柏西普联合Ex-PRESS青光眼引流器治疗NVG是安全有效的,临床总疗效优于康柏西普联合Ahmed引流阀植入治疗,可有效降低炎症因子水平。
Objective:To investigate the effect of Conbercept combined with Ex-PRESS glaucoma drainage device on patients with neovascular glaucoma(NVG).Method:A total of 110 patients(120 eyes)with NVG admitted to Gutian County Hospital and Fujian Provincial Hospital(during study)from January 2017 to December 2021 were selected.According to the random coin tossing method,they were divided into observation group and control group.There were 55 cases(58 eyes)in the control group and 55 cases(62 eyes)in the observation group.Both groups received intravitreal injection of Conbercept before operation.The observation group and the control group were treated with Ex-PRESS glaucoma drainage device implantation and Ahmed drainage valve implantation respectively.The visual acuity recovery,intraocular pressure(IOP)changes before and after treatment,treatment efficiency,aqueous humor and serum levels of vascular endothelial growth factor(VEGF),C-reactive protein(CRP)and serum interleukin-6(IL-6)were compared between the two groups.Result:(1)The visual acuity recovery rate of the observation group 3 months after treatment was higher than that of the control group,but there was no significant difference between the groups(P>0.05).(2)There was no significant difference in IOP between the two groups after treatment(P>0.05).(3)After treatment,the effective rate of surgical treatment in the observation group was higher than that in the control group,and the difference between the groups was statistically significant(P<0.05).(4)One month after treatment,the aqueous humor and serum VEGF,CRP and IL-6 in the two groups were lower than those before treatment(P<0.05),and those in the observation group were lower than those in the control group(P<0.05).Conclusion:Conbercept combined with Ex-PRESS glaucoma drainage device is safe and effective in the treatment of NVG.The total clinical effect is better than that of Conbercept combined with Ahmed drainage valve implantation,which can effectively reduce the level of inflammatory factors.
