摘要
目的:对比眶上外侧入路与翼点入路显微手术治疗急性前循环破裂动脉瘤的临床疗效。方法:回顾性分析2018年3月~2020年3月在我院进行手术治疗的76例急性前循环破裂动脉瘤患者的临床资料,按照采用翼点入路显微手术治疗或采用眶上外侧入路显微手术治疗将所有患者分为对照组(n=36)和研究组(n=40)。比较两组临床疗效、手术相关指标,并统计两组并发症发生情况。结果:研究组临床治疗总有效率明显高于对照组(P<0.05);研究组术中切口长度短于对照组,骨窗范围小于对照组,术中出血量低于对照组,且手术时间短于对照组(P<0.05);研究组并发症发生率明显低于对照组(P<0.05)。结论:与翼点入路显微手术治疗相比,眶上外侧入路行动脉瘤夹闭术治疗急性前循环破裂动脉瘤的术中切口小、骨窗范围小、出血量少,手术时间短,可增强治疗效果,减少并发症的发生风险。
Objective:To compare the clinical efficacy of supraorbital lateral approach and pterional approach in the treatment of acute anterior circulation ruptured aneurysms.Methods:The clinical data of 76 patients with acute ruptured anterior circulation aneurysms who underwent surgical treatment in our hospital from March 2018 to March 2020 were retrospectively analyzed,divided them into groups according to different surgical methods,36 patients who underwent microsurgery through the pterional approach were divided into control group,40 patients who underwent microsurgery through the lateral supraorbital approach were divided into research group.The clinical efficacy and operation related indexes were compared between the two groups,the occurrence of complications in the two groups was counted.Results:The total effective rate of research group was 97.50%,which was higher than 80.56%of control group,with a statistical significant difference(P<0.05);in research group,the incision length was shorter than that in control group,the bone window range was smaller than that in control group,the intraoperative blood loss was lower than that in control group,and the operation time was shorter than that in control group,with statistical significant differences(P<0.05);the incidence of complications in research group was 10.00%,which was lower than 27.78%in control group,with a statistical significant difference(P<0.05).Conclusion:Compared with microsurgery through the pterional approach,the supraorbital lateral approach for the treatment of acute anterior circulation ruptured aneurysms has the advantages of small incision,small bone window,less blood loss and shorter operation time,which can enhance the efficacy and reduce the risk of complications.
