摘要
目的:比较中晚期宫颈癌放疗中容积调强放疗(VMAT)与固定野调强放疗(IMRT)剂量学差异。方法:选取中晚期宫颈癌行根治性放疗的36例患者,在Eclipse计划系统中分别对每例患者进行VMAT和7野IMRT计划设计,比较两种技术的靶区适形指数、机器跳数和治疗时间、危及器官的受照剂量的差异。结果:VMAT技术D_(2%)剂量低于7IMRT技术、D_(98%)剂量高于7IMRT技术,差异均有统计学意义(P<0.05);VMAT技术靶区的均匀性指数与适形度优于7IMRT技术;机器跳数和治疗时间VMAT技术比7IMRT技术明显减少,差异有统计学意义(P<0.05)。VMAT技术中膀胱的受量V_(30)、V_(40)、V_(50)、D_(mean)低于7IMRT技术,差异有统计学意义(P<0.05)。两种技术直肠的受量V_(40)、V_(50)低于7IMRT技术,差异有统计学意义(P<0.05)。两种技术股骨头的剂量相比,差异无统计学意义(P>0.05)。结论:VMAT技术较IMRT技术可得到更好的靶区剂量分布,治疗时间明显缩短,且膀胱、直肠等危及器官能够得到很好的保护,值得在临床中开展应用。
Objective:To compare the dosimetric difference between volumetric modulated arc therapy(VMAT)and fixed field intensity modulated radiotherapy(IMRT)in advanced radiotherapy of cervical cancer.Methods:36 patients with cervical cancer were randomly selected design of volumetric modulated arc therapy and 7-field fixed field intensity modulated radiotherapy for each patient in Eclipse planning system.The conformal index of the target region exposure dose,machine hop number and treatment time between the two techniques were compared.Results:VMAT technique showed that D_(2%) dose was lower than 7IMRT technique and D_(98%)dose was higher than 7IMRT technique,with statistical significance(P<0.05).The uniformity index and conformity of the target region of VMAT technique are better than that of 7IMRT technique.The number of machine hops and treatment time of VMAT technique were significantly decreased compared with 7IMRT technique,and the difference was statistically significant(P<0.05).Distribution of bladder V_(30),V_(40),V_(50) and D_(mean) in VMAT technique were lower than those in 7IMRT technique,and the difference was statistically significant(P<0.05).The distribution of rectum V_(40) and V_(50) was lower than that of 7IMRT technique,and the difference was statistically significant(P<0.05).There was no significant difference between the two techniques in the dose of femoral head(P>0.05).Conclusion:The target dose distribution of VMAT is better than that of IMRT,the treatment time is significantly shorter,and endangering organs can also be better protected.It is worthy of clinical application.
